低浓度盐酸罗哌卡因复合舒芬太尼硬膜外自控分娩在无痛分娩中的应用效果

摘　　要：目的探究低浓度盐酸罗哌卡因复合舒芬太尼硬膜外自控分娩在无痛分娩中的临床效果。方法选取2018年9月至2020年6月本院收治的80名无痛分娩产妇作为研究对象,采用随机数字表法分为两组,各40名。观察组采用低浓度盐酸罗哌卡因复合舒芬太尼硬膜外自控分娩,对照组采用导乐仪镇痛分娩。比较两组镇痛10 min时(T1)、镇痛30 min时(T2)、镇痛120 min时(T3)、宫口全开时(T4)疼痛视觉模拟法(VAS)评分及第一产程时间、胎儿出生后Apgar评分、不良分娩结局发生率。结果观察组T1、T2、T3、T4时VAS评分均低于对照组,差异有统计学意义(P<0.05);观察组第一产程时间短于对照组,Apgar评分高于对照组,不良分娩结局发生率低于对照组,差异有统计学意义(P<0.05)。结论低浓度盐酸罗哌卡因复合舒芬太尼硬膜外自控无痛分娩可减轻产妇疼痛程度,缩短分娩时间,优化分娩结局,值得临床推广应用。Objective To investigate the clinical effect of low concentration ropivacaine hydrochloride combined with sufentanil in patient-controlled epidural delivery in painless delivery.Methods 80 painless parturients admitted to our hospital from September 2018 to June 2020 were selected as research subjects,and they were randomly divided into two groups,with 40 cases in each group.The observation group used low concentration ropivacaine hydrochloride combined with sufentanil for epidural self-control delivery,and the control group used the Doula instrument for analgesia delivery.The visual analogue system(VAS)scores of pain at 10 min(T1),30 min(T2),120 min(T3),and full opening of the uterine orifice(T4),the times of the first stage of labor,the Apgar score after birth,and the incidence of adverse delivery outcomes were compared between the two groups.Results At T1,T2,T3,T4,the VAS scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05);The length of the first stage of labor in the observation group was longer than that in the control group,the Apgar score was higher than that in the control group,and the incidence of adverse delivery outcomes was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Low concentration ropivacaine hydrochloride combined with sufentanil patient-controlled epidural delivery in painless delivery can reduce the degree of maternal pain,shorten the time of delivery,and optimize the outcome of delivery,which is worthy of clinical application.
