摘  要:目的探讨女性抑郁症患者性激素水平对血清艾司西酞普兰浓度的影响。方法选取2019年1月至2021年6月本院收治的75例女性首发抑郁症患者作为研究对象,根据患者是否处于绝经期分为对照组(生理周期正常,n=36)和研究组(绝经期,n=39),所有患者给予草酸艾司西酞普兰片治疗。比较两组血清指标[黄体生成激素(LH)、促卵泡激素(FSH)及雌二醇(E_(2))]、艾司西酞普兰浓度、焦虑和抑郁评分[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)]及不良反应发生情况;采用偏相关性分析和多元回归分析艾司西酞普兰药物浓度与性激素的关系。结果干预前及干预4、12周,研究组LH、FSH水平均高于对照组,E_(2)水平低于对照组,差异有统计学意义(P<0.05)。干预4周、12周,研究组艾司西酞普兰浓度高于对照组,差异有统计学意义(P<0.05)。偏相关分析显示,艾司西酞普兰浓度与LH、FSH水平呈正相关(r>0,P<0.05),与E_(2)水平成负相关(r<0,P<0.05)。以血清艾司西酞普兰浓度为因变量,血清LH、FSH、E_(2)为自变量,分析得到回归方程:血清艾司西酞普兰浓度=35.44+0.288×FSH-0.012×E_(2)(F=35.441,P<0.001)。两组HAMD评分时间比较差异有统计学意义(P<0.05);两组HAMD评分组间、交互比较差异无统计学意义;两组HAMA评分组间、时间、交互比较差异有统计学意义(P<0.05)。组内比较:干预4、12周,两组HAMD、HAMA评分均低于干预前,差异有统计学意义(P<0.05),干预12周,两组HAMD、HAMA评分均低于干预4周,差异有统计学意义(P<0.05)。组间比较:干预前,两组HAMD、HAMA评分比较差异无统计学意义;干预4周,研究组HAMA评分高于对照组,差异有统计学意义(P<0.05),两组HAMD评分比较差异无统计学意义;干预12周,研究组HAMD、HAMA评分均高于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论女性抑郁症患者药物干预后不同时�Objective To investigate the effect of sex hormone levels on serum escitalopram concentration in female patients with depression.Methods 75 female patients with first episode depression in our hospital from January 2019 to June 2021 were selected as the research subjects,and they were divided into control group(normal menstrual cycle,n=36)and research group(menopause,n=39)according to whether the patients were in menopause,all patients were treated with escitalopram oxalate tablets.The serum indexes(luteinizing hormone[LH],follicle stimulating hormone[FSH]and estradiol[E_(2)]),escitalopram concentration,anxiety and depression scores(Hamilton anxiety scale[HAMA],Hamilton depression scale[HAMD])and adverse reactions were compared between the two groups;partial correlation analysis and multiple regression analysis were used to analyze the relationship between escitalopram concentration and sex hormones.Results Before intervention,4 and 12 weeks after intervention,the levels of LH and FSH in the research group were higher than those in the control group,and the level of E_(2) was lower than that in the control group,and the differences were statistically significant(P<0.05).After 4 and 12 weeks of intervention,the concentration of escitalopram in the research group was higher than that in the control group,and the difference was statistically significant(P<0.05).Partial correlation analysis showed that escitalopram concentration was positively correlated with LH and FSH levels(r>0,P<0.05),and negatively correlated with E_(2) levels(r<0,P<0.05).With serum escitalopram concentration as the dependent variable and serum LH,FSH and E_(2) as the independent variables,the regression equation was obtained:serum escitalopram concentration=35.44+0.288×FSH-0.012×E_(2)(F=35.441,P<0.001).There was statistically significant difference in HAMD score between the two groups of time poins(P<0.05);there were no significant differences in HAMD scores between the two groups of group,interaction;there were statistically significant diff
