早期乳腺癌保乳术联合前哨淋巴结活检术治疗乳腺癌的临床疗效

摘　　要：目的探讨早期乳腺癌保乳术联合前哨淋巴结活检术治疗乳腺癌的临床疗效。方法选取2016年12月至2020年3月本院收治的80例乳腺癌患者作为研究对象,根据手术方式的不同分为对照组与干预组,每组40例。对照组实施乳腺癌根治术联合腋窝淋巴结清扫术治疗,干预组实施早期乳腺癌保乳术联合前哨淋巴结活检术治疗。比较两组围手术期相关指标(手术时间、术中出血量、术后引流量、腋窝拔管时间和住院时间)、美容满意度及手术前后应激反应指标[肿瘤坏死因子-α(TNF-α)和降钙素原(PCT)]、肩关节活动度、生命质量[生活质量问卷-乳腺癌模式量表(QLQ-BR23)]。结果干预组手术时间、腋窝拔管时间、住院时间均短于对照组,术中出血量、术后引流量均少于对照组,差异有统计学意义(P<0.05)。术后,两组TNF-α、PCT水平均高于对照组,但干预组低于对照组,差异有统计学意义(P<0.05)。术后6个月,两组外旋、内旋、后伸、外展及屈曲度数均小于术前,但干预组大于对照组,差异有统计学意义(P<0.05)。干预组美容总满意度为85.00%,高于对照组的55.00%,差异有统计学意义(P<0.05)。术后6个月,两组身体形象、性功能、性乐趣、未来展望、全身疗法不良反应、乳房症状、手臂症状及脱发烦恼评分均低于手术前,且干预组低于对照组,差异有统计学意义(P<0.05)。结论早期乳腺癌保乳术联合前哨淋巴结活检治疗施早期乳腺癌疗效显著,可缩短手术时间、住院时间,减少术中出血量,提高患者美容满意度及生命质量,值得临床推广应用。Objective To investigate the clinical effect of early breast conserving surgery combined with sentinel lymph node biopsy for breast cancer.Method 80 patients with breast cancer admitted to our hospital from December 2016 to March 2020 were selected as the research subjects,and they were divided into control group and intervention group according to different surgical methods,with 40 cases in each group.The control group was treated with radical mastectomy combined with axillary lymph node dissection,and the intervention group was treated with early breast conserving surgery combined with sentinel lymph node biopsy.The perioperative related indicators(operation time,intraoperative blood loss,postoperative drainage volume,axillary extubation time and hospital stay),cosmetic satisfaction,and stress response indicators(tumor necrosis factor-α[TNF-α]and procalcitonin[PCT]),shoulder joint activity,and quality of life(quality of life questionnaire-BR23[QLQ-BR23])before and after surgery were compared between the two groups.Results The operation time,axillary extubation time and hospital stay in the intervention group were shorter than those in the control group,and the intraoperative blood loss and postoperative drainage volume were less than those in the control group,and the differences were statistically significant(P<0.05).After operation,the levels of TNF-α and PCT in the two groups were higher than those in control group,and the intervention group was lower than the control group,the differences were statistically significant(P<0.05).At 6 months after operation,the degrees of external rotation,internal rotation,extension,abduction and flexion of two groups were lower than before operation,and the intervention group was higher than the control group,the differences were statistically significant(P<0.05).The total cosmetic satisfaction in the intervention group was 85.00%,which was higher than 55.00% in the control group,and the difference was statistically significant(P<0.05).At 6 months after operation,the scor
