CT引导下经皮肺穿刺活检术500例病理诊断分析

摘　　要：目的探讨CT引导下经皮肺穿刺活检术500例病理诊断分析。方法选取2018年3月至2020年3月于本院行CT引导下经皮肺穿刺活检术的500例患者作为研究对象,均经支气管镜刷检细胞学检查、支气管壁透壁肺活检术且未明确诊断,行常规术前准备及CT引导定位穿刺并获取标本。以术后病理结果为金标准,分析CT引导下经皮肺穿刺活检的穿刺成功率、确诊率、诊断效能及并发症发生率。结果术后病理结果显示,500例中明确诊断463例,确诊率为92.60%。500例患者CT引导下经皮肺穿刺活检成功率为100.00%,明确诊断肺部疾病461例,确诊率为92.20%;83例出现并发症,并发症发生率为16.60%。CT引导下经皮肺穿刺活检术对肺部疾病的诊断准确率为97.80%,灵敏度为98.70%,特异度为86.49%,阳性预测值为99.13%,阴性预测值为82.05%。结论CT引导下经皮肺穿刺活检术可较好地诊断肺部病灶,具有活检成功率高、确诊率高、准确性高等特点,临床诊断价值较高,值得临床推广应用。Objective To investigate the pathological diagnosis of 500 cases of percutaneous lung biopsy under CT guidance.Methods 500 padoi:tients who underwent CT-guided percutaneous lung biopsy in our hospital from March 2018 to March 2020 were selected as the research subjects,all patients underwent bronchoscopy for brushing cytology examination and bronchial wall transbronchial lung biopsy without clear diagnosis,and all patients underwent routine preoperative preparation and CT-guided localization puncture were performed.The postoperative pathological results were used as the gold standard,the success rate,diagnosis rate,diagnostic efficiency and complication rate of puncture were analyzed.Results Postoperative pathological results showed that the success rate of CT-guided percutaneous lung biopsy in 500 patients was 100.00%,of which 463 patients were diagnosed,and the diagnosis rate was 92.60%.The success rate of CT-guided percutaneous puncture biopsy in 500 patients was 100.00%,of which 461 patients were diagnosed with lung disease,the diagnosis rate was 92.20%;83 patients had complications,and the complication rate was 16.60%.The diagnostic accuracy of CT-guided percutaneous lung biopsy for lung diseases was 97.80%,the sensitivity was 98.70%,the specificity was 86.49%,and the positive predictive value was 99.13%,and the negative predictive value was 82.05%.Conclusion CT-guided percutaneous lung biopsy can better diagnose lung lesions,and has the characteristics of high success rate of biopsy,high diagnosis rate and high accuracy,and has high clinical diagnostic value,which is worthy of clinical application.
