益气健脾法联合肠内营养对晚期胃癌术后脾胃气虚证患者营养状态及免疫功能的影响

摘　　要：目的分析益气健脾法联合肠内营养对晚期胃癌术后脾胃气虚证患者营养状态及免疫功能的影响。方法选取2016年5月至2017年6月于本院住院的50例晚期胃癌患者作为研究对象,采用随机单盲分为观察组(中药+肠内营养)和对照组(单行肠内营养),各25例。比较两组营养状态及免疫功能指标水平。结果观察组三酰甘油(TG)、总胆固醇(TC)、血红蛋白(Hb)TG、TC、Hb水平均明显高于对照组,低密度脂蛋白(LDL)水平低于对照组,差异有统计学意义(P<0.05);观察组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)及自然杀伤细胞(NK)水平均明显高于对照组,差异有统计学意义(P<0.05);观察组免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、免疫球蛋白A(IgA)水平均明显高于对照组,差异有统计学意义(P<0.05);治疗后,观察组白细胞介素-2(IL-2)水平高于对照组,白细胞介素-6(IL-6)水平低于对照组,差异有统计学意义(P<0.05)。结论在肠内营养基础上加以益气健脾法可有效改善胃癌患者的营养不良状况与免疫功能,提高患者生命质量,促进患者康复,值得临床推广应用。Objective To analyze the effect of Yiqi Jianpi method combined with enteral nutrition on nutritional status and immune function of patients with spleen stomach qi deficiency syndrome after operation of advanced gastric cancer.Methods 50 patients with advanced gastric cancer in our hospital from May 2016 to June 2017 were selected as the research subjects,and they were randomly divided into observation group(Traditional Chinese Medicine+enteral nutrition)and control group(single enteral nutrition without Traditional Chinese Medicine),with 25 cases in each group.The nutritional status and immune function were compared between the two groups.Results The levels of triglyceride(TG),total cholesterol(TC),hemoglobin(Hb)in the observation group were significantly higher than those in the control group,and the levels of low density lipoprotein(LDL)was lower than that in the control group,the difference was statistically significant(P<0.05);the levels of CD3+,CD4+,CD8+,CD4+/CD8+and natural killer cell(NK)in the observation group were significantly higher than those in the control group(P<0.05);the levels of immunoglobulin G(IgG),immunoglobulin M(IgM)and immunoglobulin A(IgA)in the observation group were significantly higher than those in the control group(P<0.05);After treatment,the level of interleukin-2(IL-2)in the observation group was higher than that in the control group,and the level of interleukin-2(IL-6)was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion On the basis of enteral nutrition,the Yiqi Jianpi method and strengthening spleen can effectively improve the malnutrition and immune function of patients with gastric cancer,improve the quality of life and promote the rehabilitation of patients,which is worthy of clinical application.
