益气愈溃汤联合奥硝唑氯化钠注射液治疗溃疡性结肠炎临床观察

摘　　要：目的探讨益气愈溃汤与奥硝唑氯化钠注射液联合治疗溃疡性结肠炎(UC)的临床效果及对肠道菌群、凝血指标的影响。方法回顾性分析2019年6月—2020年6月于南昌市洪都中医院门诊就诊的98例UC患者的临床资料,依据治疗方案的不同将其分为观察组(49例)、参考组(49例)。观察组予以益气愈溃汤联合奥硝唑氯化钠注射液治疗,参考组予以奥硝唑氯化钠注射液治疗。对比2组治疗前后的中医证候(腹泻、腹痛、脓血便)积分;对比2组治疗前后的肠道菌群(双歧杆菌、肠杆菌、乳杆菌)水平;对比2组治疗前后的凝血指标[纤维蛋白原(FIB)、凝血酶时间(TT)、凝血酶原时间(PT)]。结果治疗2周后,观察组腹泻、腹痛、脓血便积分均较参考组低(P<0.05)。治疗2周后,观察组双歧杆菌、乳杆菌水平均较参考组高,肠杆菌水平较参考组低(P<0.05)。治疗2周后,观察组FIB含量较参考组低,TT、PT均较参考组短(P<0.05)。结论益气愈溃汤联合奥硝唑氯化钠注射液治疗UC,既能够减轻患者腹泻、腹痛及脓血便症状,调节肠道微菌群平衡,又能够改善患者的凝血功能障碍。Objective To investigate the clinical effect of Yiqi Yukui decoction combined with ornidazole in the treatment of ulcerative colitis(UC)and its influence on intestinal flora and coagulation indexes.Methods The clinical data of 98 UC patients treated in Nanchang Hongdu Hospital of Traditional Chinese Medicine from June 2019 to June 2020 were retrospectively analyzed,and they were divided into observation group(49 cases)and control group(49 cases)according to different treatment regimen.The observation group was treated with Yiqi Yukui decoction combined with ornidazole,and the control group was treated with ornidazole.The scores of traditional Chinese medicine syndromes(diarrhea,abdominal pain,pus,blood and stool)before and after treatment were compared between the two groups.The levels of intestinal flora(bifidobacteria,enterobacteria,lactobacillus)before and after treatment were compared between the two groups.The coagulation indexes(fibrinogen(FIB),thrombin time(TT),prothrombin time(PT))of the two groups before and after treatment were compared.Results After 2 weeks of treatment,the scores of diarrhea,abdominal pain,pus,blood and stool in the observation group were lower than those in the control group(P<0.05).After 2 weeks of treatment,the levels of bifidobacteria and Lactobacillus in the observation group were higher than those in the control group,while the levels of enterobacteria were lower than those in the control group(P<0.05).After 2 weeks of treatment,FIB content in observation group was lower than that in control group,TT and PT were shorter than that in control group(P<0.05).Conclusion Yiqi Yukui decoction combined with ornidazole in the treatment of UC can not only relieve diarrhea,abdominal pain,pus,blood and stool symptoms,regulate the balance of intestinal microflora,but also improve coagulation dysfunction of patients.
