纯钛烤瓷冠与钴铬合金烤瓷冠在上颌前牙种植修复中的近远期疗效比较

摘　　要：目的:比较纯钛烤瓷冠与钴铬合金烤瓷冠在上颌前牙种植修复中的近远期疗效。方法:回顾性分析2019年1月至2020年12月在该院行上颌前牙种植修复的75例患者的临床资料。两组种植体均为纯钛,依据牙冠修复材料不同分为对照组37例和研究组38例。对照组使用钴铬合金烤瓷冠,研究组使用纯钛烤瓷冠。比较两组修复后6个月时的口腔美学指标[白色美学指数(WES)、红色美学指数(PES)]水平,修复后6个月、2年的牙周健康指标[牙龈指数(GI)、探诊深度(PD)和龈沟液质量]和龈沟液指标[丙二醛(MDA)、超氧化物歧化酶(SOD)、白细胞介素-6(IL-6)]水平,以及修复后2年内不良预后发生率。结果:修复后6个月,研究组WES、PES评分均高于对照组,差异有统计学意义(P<0.05),两组牙周健康指标、龈沟液炎性因子水平比较,差异均无统计学意义(P>0.05)。修复后2年,研究组GI、PD值及龈沟液质量均低于对照组,龈沟液MDA、IL-6水平均低于对照组,SOD水平高于对照组,差异有统计学意义(P<0.05)。两组不良预后发生率比较,差异均无统计学意义(P>0.05)。结论:在上颌前牙种植修复中,纯钛烤瓷冠的美观性优于钴铬合金烤瓷冠,两种材料近期疗效及2年内不良预后发生率接近,但纯钛烤瓷冠修复2年后牙周健康指标、龈沟液指标水平均优于钴铬合金烤瓷冠。Objective:To compare short-term and long-term effects of pure titanium porcelain crown and cobalt-chromium alloy porcelain crown in implant repair of maxillary anterior teeth.Methods:The clinical data of 75 patients who underwent implant repair of maxillary anterior teeth in the hospital from January 2019 to December 2020 were retrospectively analyzed.The implants in both groups were pure titanium.According to different crown restoration materials,they were divided into control group(37 cases)and study group(38 cases).The control group used cobaltchromium alloy porcelain crown,while the study group used pure titanium porcelain crown.The oral aesthetic index levels[white aesthetic index(WES),pink aesthetic index(PES)]6 months after repair,the periodontal health index levels[gingival index(GI),probing depth(PD)and gingival crevicular fluid quality]and the gingival crevicular fluid inflammatory factor levels[malondialdehyde(MDA),superoxide dismutase(SOD),interleukin-6(IL-6)]6 months and 2 years after repair,and the incidence of poor prognosis within 2 years after repair were compared between the two groups.Results:6 months after repair,the WES and PES scores of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).There were no statistically significant differences in the periodontal health indexes and levels of inflammatory factors in the gingival crevicular fluid between the two groups(P>0.05).Further,2 years after repair,the GI and PD values as well as the gingival crevicular fluid quality of the study group were lower than those of the control group,the levels of MDA and IL-6 in gingival crevicular fluid were lower than those of the control group,the level of SOD was higher than that of the control group,and the differences were statistically significant(P<0.05).Conclusions:In the implant repair of maxillary anterior teeth,the aesthetics of pure titanium porcelain crown is better than that of cobaltchromium alloy porcelain crown.The short-term effi
