地西他滨联合维奈克拉治疗老年白血病的近期疗效和安全性

摘　　要：目的观察地西他滨联合维奈克拉治疗老年白血病的临床疗效和安全性。方法选取2018年2月—2020年2月苏州永鼎医院收治的老年白血病患者50例,根据随机数字表法分为观察组(n=25)和对照组(n=25)。对照组采用地西他滨药物联合预激方案治疗,观察组采用地西他滨联合维奈克拉治疗。比较2组患者临床疗效、骨髓抑制及恢复时间、不良反应。结果观察组患者总有效率为96.00%,高于对照组的76.00%(χ^(2)=4.153,P=0.042);观察组患者血红蛋白恢复时间、低谷期持续时间均短于对照组,白细胞、血小板恢复时间、低谷期持续时间均长于对照组(P<0.05或P<0.01);观察组患者不良反应总发生率为12.00%,低于对照组的36.00%(χ^(2)=3.947,P=0.047)。结论地西他滨联合维奈克拉治疗老年白血病具有较好的临床效果,可改善骨髓抑制及恢复时间,不良反应发生率低,值得推广和应用。Objective To observe the clinical efficacy and safety of decitabine combined with venetoclax in the treatment of senile leukemia.Methods 50 elderly leukemia patients admitted to Suzhou Yongding Hospital from February 2018 to February 2020 were selected and divided into observation group(n=25)and control group(n=25)according to random number table method.The control group was treated with decitabine combined with pre-shock therapy,and the observation group was treated with decitabine combined with venetoclax.The clinical efficacy,myelosuppression,recovery time and adverse reactions of the two groups were compared.Results The total effective rate of observation group was 96.00%,which was higher than that of control group(76.00%)(χ^(2)=4.153,P=0.042).The recovery time of hemoglobin and the duration of nadir in observation group were shorter than those in control group,while the recovery time of white blood cells and platelets and the duration of nadir were longer than those in control group(P<0.05 or P<0.01).The total incidence of adverse reactions of observation group was 12.00%,which was lower than that of control group(36.00%)(χ^(2)=3.947,P=0.047).Conclusion Decitabine combined with venetoclax in the treatment of senile leukemia has good clinical effect,can improves bone marrow suppression and recovery time,and has a low incidence of adverse reactions,so it is worthy of promotion and application.
