电针八髎穴联合自拟益阳愈溃汤加减治疗溃疡性结肠炎的疗效及对肠道屏障功能的影响

摘　　要：目的:探讨电针八髎穴联合自拟益阳愈溃汤加减治疗溃疡性结肠炎(UC)的疗效及对肠道屏障功能的影响。方法:选取2019年8月-2021年6月期间于我院治疗的UC患者80例,通过随机数字表法将纳入患者随机分为对照组和联合组,每组40例,对照组予以美沙拉秦缓释颗粒口服治疗,联合组在此基础上予以电针八髎穴联合自拟益阳愈溃汤加减治疗。比较两组治疗前后中医症状积分、粪便钙卫蛋白(FC)、C反应蛋白(CRP)以及红细胞沉降率(ESR)水平、肠道粘膜屏障功能变化,并评价临床疗效。结果:治疗后,联合组治疗总有效率为92.50%,高于对照组的75.00%(P<0.05);联合组腹泻、食少纳差、腰酸膝软及形寒肢冷等中医症状积分显著低于对照组(P<0.05);联合组FC、CRP、ESR水平显著低于对照组(P<0.05);联合组D-乳酸、内毒素、黏蛋白2水平明显低于对照组(P<0.05)。结论:电针八髎穴联合自拟益阳愈溃汤加减治疗可有效增强对UC患者的临床治疗效果,其机制包括抑制炎症反应,改善肠道粘膜屏障功能。Objective To explore the curative effect of electroacupuncture at Baliao acupoint with Yiyang Yukui decoction on ulcerative colitis(UC)and its influences on intestinal barrier function.Methods A total of 80patients with UC treated in the hospital were selected from August 2019to June 2021.According to the random number table method,they were divided into the control group and the observation group,with 40cases in each group.The control group was given oral mesalazine sustained-release granules,while the observation group was additionally treated with electro-acupuncture at Baliao acupoint and Yiyang Yukui decoction.Comparison of the TCM symptom scores,fecal calprotectin(FC),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)levels,and changes in intestinal mucosal barrier function before and after treatment between the two groups,and to evaluate the clinical efficacy.Results:The total response rate of treatment in the observation group was significantly higher than that in the control group(92.50%vs 75.00%)(P<0.05).The scores of TCM symptoms(diarrhea,poor appetite,soreness and weakness of waist and knees,cold limbs)in the observation group were significantly lower than those in the control group(P<0.05),levels of FC,CRP and ESR were significantly lower than those in the control group(P<0.05),and levels of D-lactate,endotoxin and mucin 2were significantly lower than those in the control group(P<0.05).Conclusion Electro-acupuncture at Baliao acupoint with Yiyang Yukui Decoction can effectively enhance the clinical effect on UC patients,and its mechanisms include inhibiting inflammatory response and improving intestinal mucosal barrier function.
