摘　　要：目的:探讨黄连温胆汤联合多奈哌齐治疗痰浊阻窍型轻度阿尔茨海默病(AD)的临床疗效。方法:选取2021年2月~6月于某三级老年专科医院住院的痰浊阻窍型轻度AD患者54例作为研究对象,随机分为对照组和观察组各27例,对照组予以盐酸多奈哌齐片口服(1次/qn),观察组在此基础上加用黄连温胆汤冲服(每日1剂),两组均连续服用12周后观察疗效。比较治疗前后简易智能精神状态检查量表(MMSE)、蒙特利尔认知评估量表(MoCA)、韦氏记忆量表逻辑记忆(LMS)、AD认知评定量表(ADAS-Cog)的评分及肿瘤坏死因子α(TNF-α)、超氧化物歧化酶(SOD)、丙二醛(MDA)水平变化。结果:观察组的治疗有效率为80.77%,显著高于对照组的46.16%(P<0.001);观察组SOD、MMSE、MoCA、LMS水平比较对照组显著升高(P<0.05),TNF-α、MDA、ADAS-Cog水平较对照组显著降低(P<0.05)。结论:黄连温胆汤联合多奈哌齐片干预痰浊阻窍型轻度AD有利于提高患者认知能力及记忆,抑制炎症反应,调节机体抗氧化应激功能。Objective:To explore the clinical efficacy of Huanglian Wendan Decoction combined with donepezil in the treatment of mild Alzheimer's disease(AD)with phlegm obstructing orifices type.Methods:54patients with mild AD of phlegm obstructing orifices type were hospitalized in a tertiary geriatric hospital from February to June 2021were selected as the research objects.They were randomly divided into control group and observation group,with 27cases in each group.The control group was given donepezil hydrochloride tablets orally(1time per day).On this basis,the observation group was given Huanglian Wendan Decoction(1dose per day),and the curative effect was observed after 12weeks of continuous administration.Before and after treatment,The levels of Mini-Mental State Examimation(MMSE),Montreal Cognitive Assessment(MoCA),Webster's Memory Scale Logical memory(LMS),Alzheimer's Disease Assessment Scale-Cognitive section(ADAS-cog),Tumor Necrosis Factor(TNF-α),Superoxide Dismutase(SOD)and Malondialdehyde(MDA)were measured.Results:The effective rate of the observation group was 80.77%,significantly higher than 46.16%of the control group(χ2=26.426,P<0.001).MMSE,MoCA,LMS and SOD in the observation group were significantly higher than those in the control group(P<0.05),ADAS-cog and TNF-α、MDA level were significantly lower than those in the control group(P<0.05).Conclusion:Huanglian Wendan Decoction Combined with Donepezil tablets can improve the cognitive ability and memory of patients with mild AD,inhibit inflammation and regulate the body's anti-oxidative stress function.
