香砂六君子汤联合中药足浴治疗乳腺癌术后化疗相关性恶心呕吐的临床研究

摘　　要：目的 观察香砂六君子汤联合中药足浴治疗乳腺癌术后化疗相关性恶心呕吐(CINV)的临床效果。方法 前瞻性选取2021年1月~2022年1月解放军总医院第一医学中心接受化疗治疗后出现CINV的64例乳腺癌术后住院患者,采用随机数表法分为治疗组和对照组各32例,对照组按常规西医标准给予格拉司琼联合地塞米松治疗,治疗组在对照组基础上联合香砂六君子汤口服及中药足浴。观察比较两组的恶心呕吐症状改善情况及不良反应发生情况。结果 治疗后治疗组总有效率为90.6%(28/32),明显高于对照组的65.6%(21/32),差异有统计学意义(χ~2=8.537,P <0.05)。治疗后两组Karnofsky氏(KPS)评分相对治疗前有明显改善,差异具有统计学意义(P <0.05),且治疗组比对照组改善更显著,比较有统计学意义(P <0.05)。治疗组患者不良反应发生率低于对照组,差异有统计学意义(χ~2=6.488,P <0.05)。结论 在西医治疗基础上加用香砂六君子汤口服及中药足浴治疗,能够减少乳腺癌术后CINV的发生,有效提升疗效,改善患者的生活质量,降低不良反应发生率。Objective To evaluate the effect of Xiangsha Liujunzi Decoction combined with footbathing with traditional Chinese medicine in chemotherapy patients on the improvement of CINV after breast cancer operation.Methods A total of 64 patients undergoing chemotherapy after breast cancer surgery from January 2021 to January 2022 were randomly divided into control group and treatment group,with 32 patients in each group.The control group received granisetron+dexamethasone intravenous drip,while the treatment group received Xiangsha Liujunzi Decoction combined with footbathing with traditional Chinese medicine orally in addition to the treatment given to the control group.The symptoms of nausea and vomiting were compared between the two groups.Results The efficacy of treatment group relative to control group was significantly improved in CINV.The total effective rate of treatment group 90.6%(28/32),while the control group was 65.6%(21/32),which was statistically significant(P<0.05).After treatment,the two groups showed significant improvements in KPS scores before treatment.There was a statistically significant difference,and the improvement in the above two scores was more significant in treatment group relative to control group with statistical significance(P<0.05).The incidence rate of adverse response in treatment group was significantly lower than that in control group.Conclusion The use of Xiangsha Liujunzi Decoction combined with footbathing using traditional Chinese medicine in chemotherapy patients after breast cancer operation with CINV is a feasible treatment plan.It significantly improves the quality of life of patients,increases treatment efficacy,and reduces the incidence of adverse reactions.
