摘　　要：目的通过对比3种乳腺根治术标本的取材方式和固定时间,摸索制备高质量乳腺根治术标本切片的最佳固定方法。方法随机收集上午、下午接收乳腺根治术标本50例,上午每例标本各取一块肿物、腺体组织和淋巴结组织,固定9 h后放入脱水机;下午每例肿物、腺体和淋巴结各取2块,1块固定4 h后放入脱水机,另1块固定28 h后放入脱水机。各组织常规脱水、石蜡包埋、切片后,进行HE染色、E-cadherin酶标法免疫组织化学检测和Her-2荧光原位杂交染色。比较3种固定方法的制片质量,探讨乳腺根治术标本最佳固定方法。结果固定4 h即开始脱水的肿物和腺体组织的切片不完整发生率显著高于固定9 h和28 h;固定4h和9h的淋巴结切片不完整发生率高于固定28 h;固定4 h、9 h、28 h肿物、腺体HE染色质量无明显差异,但固定9 h与28 h的肿物、腺体E-Cadherin酶标法免疫组织化学正常率高于固定4 h,固定28 h淋巴结染色质量高于固定4 h与9 h;固定4 h肿物Her-2荧光原位杂交成功率低于固定9 h与28 h。结论乳腺根治术标本的肿物与腺体固定9 h后即可放入脱水机,淋巴结则需延长固定时间至28 h后放入脱水机。Objective To explore the optimal fixation method for preparing high-quality breast radical surgery specimens by comparing the sampling methods and fixation times of three breast radical surgery specimens.Methods 50 breast radical surgery specimens were randomly collected in the morning or in the afternoon.In the morning,a lump,a glandular tissue and a lymph node tissue were taken from each specimen,fixed for 9 h and then put into a dehydrator;in the afternoon,two pieces of tissues were taken from each lump,glandular and lymph node,one piece was fixed for 4 h and then put into a dehydrator,and the other piece was fixed for 28 h and then put into a dehydrator.After routine dehydration,paraffin embedding and sectioning,HE staining,enzyme labeling immunohistochemical detection of E-cadherin and Her-2 fluorescence in situ hybridization staining were performed.The quality of the sections prepared by three fixation methods was compared to explore the optimal fixation method for breast radical surgery specimens.Results The incidence of incomplete sections of lumps and glandular tissues that began dehydration after 4 h of fixation was significantly higher than that of those that underwent 9 h or 28 h of fixation;the incidence of incomplete sections of lymph nodes fixed for 4 h or 9 h was higher than that of those fixed for 28 h;the HE staining quality of lumps and glandular tissues fixed for 4 h,9 h or 28 h was not significantly different,but the rate of normal E-cadherin enzyme-labeled immunohistochemical staining of lumps and glandular tissues fixed for 9 h or 28 h was higher than that of those fixed for 4 h;the staining quality of lymph nodes fixed for 28 h was higher than that of those fixed for 4 h or 9 h;the success rate of Her-2 fluorescence in situ hybridization of lumps fixed for 4 h was lower than that of those fixed for 9 h or 28 h.Conclusion The lumps and glandular tissues from the specimens of breast radical surgery can be put into a dehydrator after being fixed for 9 h,while lymph nodes need to be fixed for
