眶隔法重睑成形术手术前后睑板前脂肪筋膜层组织结构差异临床超声观察

摘　　要：目的 探究眶隔法重睑成形术睑板前组织结构的变化。方法 纳入自2019年12月至2021年12月,湖南师范大学附属第一医院(湖南省人民医院)整形激光美容科行眶隔法重睑成形术治疗110例先天性单睑患者,招募同样数量先天性重睑志愿者。眶隔法重睑成形术患者术前为A组,术后6个月为B组,先天性重睑志愿者为C组。通过超声观察眶隔法重睑成形术术前、术后与先天性重睑的睑板前脂肪筋膜层组织结构差异。结果 超声观察显示,3组睑板前脂肪筋膜层均分为两层,分别是浅面的疏松脂肪筋膜层和深面的致密筋膜层。3组致密筋膜层紧贴睑板表面且非常薄。疏松脂肪筋膜层厚度:A组与B组、A组与C组比较,差异均有统计学意义(P<0.05);B组与C组比较,差异无统计学意义(P>0.05)。行眶隔法重睑成形术的患者术后效果评价:满意98例,基本满意9例,不满意3例。结论 眶隔法重睑成形术术后睑板前脂肪筋膜层组织结构接近先天性重睑的睑板前脂肪筋膜层组织结构,提示眶隔法重睑成形术能够改变睑板前组织结构,使之接近先天性重睑,是其取得良好临床效果的解剖基础之一。Objective To observe the changes of tissue structure after double eyelid blepharoplasty with the orbital septum method.Methodss From December 2019 to December 2021,this study included 110 patients who underwent double eyelid blepharoplasty with the orbital septum method and 110 congenital double eyelid volunteers.They divided into three groups:preoperative eyelid ultrasound images in group A,postoperative eyelid ultrasound images in group B,and congenital double eyelid volunteers in group C.Histological differences of pretarsal fat fascia before and after double eyelid blepharoplasty with the orbital septum method and congenital double eyelid were observed by ultrasound.ResultsUltrasonography showed that the pretarsal fat fascia layer was divided into two layers.In Group A,B,and C,the dense layer was close to the surface of tarso.The pretarsal loose fat fascia layer thickness in group A were significantly thicker than those in group B and C(P<0.05),and there was no significant difference between group B and C(P>0.05).Postoperative evaluation:98 cases were satisfied,9 cases were basically satisfied and 3 cases were unsatisfied.Conclusion After double eyelid blepharoplasty with the orbital septum method,the histological structure of pretarsal fat fascia is similar to that of congenital double eyelid,which suggests that double eyelid blepharoplasty with the orbital septum method can change the structure of pretarsal fat fascia and make it similar to congenital double eyelid,which is one of the anatomical bases for its good clinical effect.
