重睑成形术在肿泡眼中的临床应用

摘　　要：目的 探讨对上睑臃肿即肿泡眼患者行重睑成形术的临床效果。方法 自2020年6月至2022年6月,吉林大学第一医院整形美容外科对127例单睑伴上睑臃肿患者行切开法重睑成形术。针对其解剖特点,术中以各层次减容为主,切除多余皮肤、眼轮匝肌,减少眼轮匝肌后脂肪(retro-orbicularis oculus fat,ROOF)及眶隔脂肪容量,如有脱垂泪腺给予复位,完成重睑成形术。结果 127例患者术后切口均一期愈合。所有患者获随访6~24个月(平均12个月),术后效果满意,肿泡眼外观纠正,无严重并发症发生。结论 根据肿泡眼患者上睑形态臃肿的原因对各层次组织进行充分去除,能取得良好的效果,形成轻薄重睑。Objective To investigate the clinical effect of blepharoplasty with volume reduction in patients with hypertrophic eyelids.Methods From June 2020 to June 2022,127 patients with hypertrophic upper eyelids who underwent full-incision upper blepharoplasty were involved in this study.According to the anatomical characteristics,the volume reduction was mainly carried out at various layers during the operation,including excised excess skin,orbicularis oculi muscle,reduced retro-orbicularis oculus fat(ROOF)and orbital septum fat capacity,as well as lacrimal gland refixation.Results All 127 patients were followed up for 6 to 24 months(mean 12 months)and had primary healing after surgery.The results were satisfactory and the appearance of"swollen eyes"was corrected without adverse complications.Conclusion According to the causes of the swollen upper eyelid,all layers of soft tissue should be fully removed to achieve good results and form thin and double eyelids.
