基于脾失备化探讨脾瘅火热病机

摘　　要：脾瘅是因过食肥甘而致中满内热,并且以口甘为主症的一种代谢性疾病前状态。脾瘅病位在脾,病理基础在于火热熏灼。本文基于《黄帝内经》“备化”一词探究脾瘅火热根源,提出“备多-化少”这一病理过程为火热之源。脾瘅之始,因“备多”而纳化失衡,积而化火;随着火邪发展,因“化少”而精微不运,郁而化火。脾瘅之火亦可传至五脏,而引起不同的临床症状。临证以恢复脾之备化平衡为要,截断火邪发生、发展,同时可根据火热传变,分脏论治,有助于实现脾瘅火热的精准治疗。Spleen pyretic disease is a pre-morbid state of metabolic disease caused by excessive consumption of fatty and sweet foods,resulting in internal heat and fullness in the middle,and the main clinical manifestation is sweet mouth.The location of Spleen pyretic disease lies in Spleen,and the basis of the pathology lies in fire-heat fever.This study investigated the source of fire-heat in Spleen pyretic disease based on the term‘preparation and transformation’in Yellow Emperor s Inner Canon,and proposed that the pathological process of‘more preparation-less transformation’in Spleen was the source of fire-heat.At the beginning of Spleen pyretic disease,the‘more preparation’leads to the imbalance of absorption and transformation,which causes the accumulation of fire;as the fire evolves,the‘less transformation’leads to the lack of transportation of the essence,which results in the transmission of fire.The fire in Spleen can also spread to five Zang-organs and cause different clinical symptoms.The purpose of clinical treatment should always be to restore the balance of Spleen preparation,so as to prevent the occurrence and development of fire pathogens;at the same time,it should treat the fire according to its transmission to the organs,which can help to realize the precise treatment of fire-heat.
