腹腔镜直肠全系膜切除治疗直肠癌的临床研究

摘　　要：目的 观察对直肠癌患者予以腹腔镜直肠全系膜切除术的手术效果。方法 观察对象选自于2018年10月-2022年10月就诊于本院的直肠癌患者80例,随机方法分组观察,予以腹腔镜直肠全系膜切除术治疗的40例患者纳入研究组,予以开腹手术治疗的40例患者纳入对照组,对手术效果(手术效果、并发症、炎性因子水平、生活质量)进行对比和分析。结果 研究组手术时间、术中出血量、切口长度、术后排气时间、淋巴结清扫数、住院时间比较对照组更佳(P<0.05);研究组并发症比较对照组更低(P<0.05);炎性因子水平(CRP、IL-6),治疗前两组比较(P>0.05);治疗后研究组炎性因子水平比较对照组更高(P<0.05);生活质量,治疗前两组比较(P>0.05);治疗后研究组比较对照组更佳(P<0.05)。结论 对直肠癌患者予以腹腔镜直肠全系膜切除术治疗效果突出,可优化手术指标,降低创伤性,可使并发症减少,可减轻炎性反应,利于患者生活质量改善,建议推广。Objective To observe surgical effect of laparoscopic total mesorectal resection for patients with rectal cancer.Methods The paper chose 80 rectal cancer patients in our hospital from October 2018 to October 2022 as observation objects,and divided them into groups randomly,40 patients in study group were treated with laparoscopic total mesorectal resection,and 40 cases in control group with open surgery.Surgical effect(surgical effect,complications,inflammatory factor levels and life quality)was compared and analyzed.Results Study group had better surgical time,intraoperative bleeding,incision length,postoperative exhaust time,lymph node clearance and length of stay than control group(P<0.05).Complications in study group were lower than control group(P<0.05).Levels of inflammatory factors(CRP,IL-6)comparison between two groups before treatment showed(P>0.05).After treatment,levels of inflammatory factors in study group were higher than control group(P<0.05).Life quality comparison between two groups before treatment showed(P>0.05).After treatment,that of study group was better than control group(P<0.05).Conclusion Laparoscopic total mesorectal resection can achieve significant effect in treatment of rectal cancer patients,can optimize surgical indicators,reduce trauma and complications,alleviate inflammatory reactions and improve life quality of patients,which is worthy of promotion.
