摘　　要：目的总结先天性牙龈瘤(CE)的CT表现,提高对本病的认识。资料与方法回顾性分析2017年1月—2022年9月广州市妇女儿童医疗中心5例经病理确诊的CE的诊疗记录及CT图像,观察肿瘤在患儿出生后的形态变化及瘤体的密度、强化特点。结果5例女性患儿共6枚牙龈肿瘤,瘤体呈息肉状或不规则状,以蒂状结构与牙龈相连,于出生后首月增大。CT平扫及增强扫描显示,牙龈肿瘤外周部分密度始终高于中央部分。动脉期肿瘤外周部分呈中度(4枚)及明显强化(2枚);中央部分呈无强化(3枚)、轻度(2枚)或中度强化(1枚)。静脉期瘤体持续强化,原动脉期中央无强化区出现强化。结论女性新生儿牙龈外生性肿瘤需考虑CE可能。CT平扫及增强扫描牙龈肿瘤中央部分密度始终低于外周部分、动脉期中央部分可呈无强化为CE相对特异的CT表现。Purpose To summarize the CT manifestations of congenital epulis(CE)so as to improve the understanding of CE.Materials and Methods Retrospective analysis was conducted on diagnostic and therapeutic records,as well as CT images,and a total of five patients with CE at Guangzhou Women and Children's Medical Center from January 2017 to September 2022 were selected.The morphological changes of the masses after birth and the density as well as enhancement characteristics of CE were observed.Results There were 5 CE female children with 6 oral masses.All six gingival masses were polypoid or irregular,and were connected to the gingiva via a pedunculated structure,and increased in size within the first month after birth.In plain and enhanced phases,the density of the peripheral part of the gingival mass was always higher than those of the central part.The peripheral tumors in arterial stage were moderate(4 pieces)and significantly enhanced(2 pieces).The central part was no reinforcement(3 pieces),mild(2 pieces)or moderate reinforcement(1 piece).The tumor continued to strengthen in the venous stage,and the central unstrengthened area appeared in the primary artery stage.Conclusion The possibility of CE should be considered in female neonates with gingival exophytic tumors.In plain and enhanced phases,the density of the central part of the gingival mass is always lower than that of the peripheral part,and the central part of the tumor in the arterial phase may show no enhancement,which are relatively specific imaging findings of CE.
