摘　　要：目的:探讨口腔扁平苔藓(oral lichen planus,OLP)患者血清中γ干扰素(IFN-γ)、白细胞介素4(IL-4)表达水平对免疫应答的调节作用。方法:选取2019年1月—2021年12月首都医科大学附属北京口腔医院王府井部收治的83例OLP患者为实验组,另纳入同期来院的61例健康志愿者为对照组。比较2组患者血清中IFN-γ、IL-4及免疫功能指标的表达情况,并分析实验组IFN-γ、IL-4表达水平与免疫功能指标的关系。采用SPSS 22.0软件包对数据进行统计学分析。结果:实验组IFN-γ、IL-4水平显著高于对照组(P<0.05)。实验组网纹型OLP、糜烂型OLP患者的IFN-γ水平对比差异无统计学意义(P>0.05),实验组糜烂型OLP患者的IL-4水平显著高于网纹型OLP患者(P<0.05)。实验组CD3^(+)、CD4^(+)、CD16^(+)+CD56^(+)、CD4^(+)/CD8^(+)水平显著低于对照组(P<0.05),实验组CD8^(+)、CD19^(+)水平显著高于对照组(P<0.05)。实验组IgG、IgM、CH50水平显著高于对照组(P<0.05),实验组IgA、C3、C4水平显著低于对照组(P<0.05)。Pearson相关性分析显示,实验组IFN-γ水平与CD16^(+)+CD56^(+)、CD4^(+)/CD8^(+)呈负相关(P<0.05);实验组IL-4水平与CD16^(+)+CD56^(+)、CD4^(+)/CD8^(+)呈负相关(P<0.05),实验组IL-4水平与IgG呈正相关(P<0.05)。结论:OLP患者血清中IFN-γ、IL-4异常高表达且糜烂型OLP患者IL-4表达更高,OLP患者细胞免疫及体液免疫功能紊乱且与CD16^(+)+CD56^(+)、CD4^(+)/CD8^(+)、Ig G免疫应答调节有关。PURPOSE:To investigate the regulation effect of interferon-γ(IFN-γ)and interleukin-4(IL-4)in the serum of patients with oral lichen planus(OLP)on immune response.METHODS:A total of 83 OLP patients in Wangfujing Branch of Beijing Stomatological Hospital,Capital Medical University from January 2019 to December 2021 were selected as the experimental group,and another 61 healthy volunteers who came to the hospital during the same period were included as the control group.The expression levels of IFN-γand IL-4 in the serum and the expression of immune function indexes of the two groups were compared.The relationship between the expression levels of IFN-γ,IL-4 and immune function indicators in the experimental group was analyzed.SPSS 22.0 software package was used to analyze the data.RESULTS:The levels of IFN-γand IL-4 in the experimental group were significantly higher than those in the control group(P<0.05).There was no significant difference in the levels of IFN-γbetween patients with reticulated OLP and erosive OLP in the experimental group(P>0.05).The level of IL-4 in patients with erosive OLP in the experimental group was significantly higher than that in patients with reticulated OLP(P<0.05).The levels of CD3+,CD4,CD16++CD56+and CD4+/CD8+in the experimental group were significantly lower than those in the control group(P<0.05),the levels of CD8+and CD19+in the experimental group were significantly higher than those in the control group(P<0.05).The levels of IgG,IgM and CH50 in the experimental group were significantly higher than those in the control group(P<0.05),and the levels of IgA,C3 and C4 in the experimental group were significantly lower than those in the control group(P<0.05).Pearson correlation analysis showed that the level of IFN-γin the experimental group was negatively correlated with CD16++CD56+and CD4+/CD8+(P<0.05),the level of IL-4 in the experimental group was negatively correlated with CD16++CD56+and CD4+/CD8+(P<0.05),and the level of IL-4 in the experimental group was positively correl
