摘 要:目的 分析广西人乳头瘤病毒(HPV)各基因型分布情况,以及HPV感染清除的影响因素,为今后人乳头瘤病毒感染及相关疾病的防治提供参考依据。方法 回顾性分析2021年3月至2021年11月在广西壮族自治区人民医院就诊的3 851例患者脱落细胞或分泌物样本HPV基因分型检测的结果,进一步追踪检测137例患者常规治疗3个月后的HPV基因分型,并分析其清除情况。结果 3 851例受检者中,HPV总阳性率为41.24%,男性为45.31%、女性为40.26%。1 588例阳性患者共检出2 362个HPV基因型,高危型占64.78%,常见的基因型为HPV52、HPV6、HPV58、HPV16、HPV53。在宫颈HPV感染组,高危型占74.72%,常见的基因型为HPV52、HPV58、HPV16。在生殖器疣组,高危型占41.16%,感染常见的基因型为HPV6、HPV11、HPV52。患者常规治疗3个月后总体HPV清除率为33.58%,在各年龄段的清除率差异无统计学意义(43.75%vs 27.45%vs 40.63%vs 30.77%vs 33.33%;χ^(2)=2.386,P=0.665),在生殖器疣感染者中,不同性别患者HPV清除率差异无统计学意义(57.14%vs 45.45%;χ^(2)=0.396,P=0.529),女性生殖器疣患者HPV清除率高于无疣宫颈感染者(45.45%vs 13.46%;χ^(2)=4.131,P=0.042),单一HPV感染组清除率高于双重和多重感染组(41.76%vs 18.75%vs 14.29%;χ^(2)=8.221,P=0.016),在单一HPV感染中,高危型和低危型的清除率差异无统计学意义(38.18%vs 50.00%;χ^(2)=1.241,P=0.265)。结论 广西人感染HPV基因型基本符合亚洲人群分布规律,宫颈HPV感染较生殖器疣更易形成持续感染,多重感染较单一感染更难清除。Objective To learn type-specific distribution of human papillomavirus(HPV)and the influencing factors of HPV infection clearance in Guangxi,and to provide reference for future clinical diagnosis and treatment of HPV infection and related diseases.Methods We retrospectively analyzed the results of HPV genotyping detection in exfoliated cells or secretion samples of 3851 patients who were treated in the People's Hospital of Guangxi Zhuang Autonomous region from March 2021 to November 2021.137 patients were tested again after conventional treatment for 3 months,and the clearance was analyzed.Results Among the 3851 subjects,the total positive rate was 41.24%.The infection rates of HPV in male and female patients were 45.31%and 40.26%,respectively.A total of 2362 HPV genotypes were detected in 1588 positive patients,of which the high-risk genotypes of HPV accounted for 64.78%.The common genotypes of HPV infection were HPV52,HPV6,HPV58,HPV16 and HPV53.In the cervical HPV infection group and genital warts group,the high-risk types accounted for 74.72%and 41.16%,respectively,and the common genotypes for HPV infection were HPV(52,58,16)and HPV(6,52,16),respectively.The HPV clearance rate was 33.58%after 3 months of routine treatment in HPV-infected patients,and there was no significant difference in the HPV infection clearance rate among different age groups(43.75%vs 27.45%vs 40.63%vs 30.77%vs 33.33%;χ^(2)=2.386,P=0.665).In patients with genital warts,there was no significant difference in HPV clearance rate between in male and female(57.14%vs 45.45%;χ^(2)=0.396,P=0.529).The clearance rate of HPV was higher in genital warts disease than in cervical infection disease(45.45%vs 13.46%;χ^(2)=4.131,P=0.042),and higher in HPV single infection group than in double infection group or triple infection group(41.76%vs 18.75%vs 14.29%;χ^(2)=8.221,P=0.016).There was no significant difference in the clearance rate between the high-risk group and the low-risk group in a single HPV infection(38.18%vs 50.00%;χ^(2)=1.241,P=0.265).Conc
