摘　　要：目的分析大连市结膜炎患者腺病毒感染型别分布情况及流行病学特征。方法选取2017—2021年在大连市3家医院眼科就诊的急性结膜炎患者作为研究对象,对其进行问卷调查,采集其眼拭子标本,采用Real-time PCR方法进行核酸检测。结果678例急性结膜炎患者的眼拭子样本中,腺病毒阳性219份。其中,腺病毒3型52份,8型109份,19/37型47份,同时分离出3型和8型1份,未分型10份。男性多于女性,20~<40岁占比最高。腺病毒感染引起的急性结膜炎患者主要临床表现为眼结膜充血、异物感、眼刺痛、畏光流泪、肿胀、眼睑沉重、烧灼感、黏液分泌物、水样分泌物等。不同型别腺病毒阳性者发生眼刺痛(χ^(2)=14.582)、畏光流泪(χ^(2)=6.375)的频率比较,差异有统计学意义(P<0.05)。结论2017—2021年大连市急性结膜炎致病病原感染的主要病原体是腺病毒。腺病毒8型最多,其次是3型和19/37型,在今后的监测中应加强对腺病毒的检测。Objective To know the prototype distribution and epidemiological characteristics of acute conjunctivitis caused by adenovirus in Dalian City.Methods Questionnaires and conjunctival swabs were collected from outpatients with acute conjunctivitis from 2017 to 2021 in three hospitals in Dalian City.The Real-time PCR method was used for nucleic acid detection.Results Of 678 samples of acute conjunctivitis,219 samples were positive for adenovirus,including 52 for type 3,109 for type 8,47 for type 19/37,1 for type 3 and 1 for type 8,and 10 for ungenotyped.There were more males than females,and the positive rate was highest in the 20~<40 age group.Acute conjunctivitis caused by adenovirus infection mainly manifests as conjunctival hyperemia,foreign body sensation,eye tingling,photophobia,swelling,eyelid heaviness,burning sensation,mucus secretion and watery secretion,etc.There was statistically significant difference in the frequency of eye tingling(χ^(2)=14.582)and photophobia(χ^(2)=6.375)among different types of adenovirus-positive patients(P<0.05).Conclusion Adenovirus was the main viral pathogen of acute conjunctivitis from 2017 to 2021 in Dalian City.Adenovirus type 8 is the most common,followed by type 3 and type 19/37.Adenovirus detection should be strengthened in future surveillance.
