妇女绝经期HPV感染情况及影响因素分析

摘　　要：目的探讨绝经期妇女人乳头瘤病毒(HPV)感染情况并分析其影响因素。方法选取2021年1月—2022年1月在桐庐县第一人民医院妇科门诊就诊且接受检查的289例绝经期妇女为调查对象,采用HPV分型检测试剂盒检测其HPV感染情况,根据检测结果将其分为感染组与非感染组。采用单因素及多因素logistic回归分析绝经期妇女HPV感染的影响因素。结果289名接受HPV筛查的绝经期妇女中,感染78例,占26.99%。经单因素分析,两组感染者的年龄、吸烟情况、居住地、文化程度、首次性生活年龄、避孕措施、性伴侣人数、宫颈糜烂、免疫系统疾病以及个人卫生习惯差异有统计学意义(P<0.05)。经多因素logistic回归分析,年龄大、吸烟、居住地是农村、小学及以下文化程度、性伴侣人数≥2人、宫颈糜烂、免疫系统疾病以及个人卫生习惯不良是绝经期妇女发生HPV感染的风险因素(P<0.05)。结论绝经期妇女HPV感染率处于较高水平,且受多种因素影响。应加大对绝经期妇女HPV相关知识的宣传力度。Objective To investigate the infection of human papillomavirus(HPV)of menopausal women and analyze the influencing factors.Methods 289 postmenopausal women who adimitted to Gynecology Clinic and received gynecological examination from January,2021 to January,2022 were selected as the subjects.HPV infection was detected with HPV typing test kit,and they were divided into infection group and non-infection group according to the test results.Single factor and multi-factor logistic regression were used to analyze the influencing factors of HPV infection in postmenopausal women.Results Among 289 menopausal women who received HPV screening,78 were infected,accounting for 26.99%.By single factor analysis,there were significant differences between the two groups in age,smoking,residence,education level,age of first sexual life,contraceptive measures,number of sexual partners,cervical erosion,immune system diseases and personal hygiene habit(P<0.05).Multivariate logistic regression analysis showed that old age,smoking,residence in rural areas,primary school and below,≥2 sexual partners,cervical erosion,immune system diseases and poor personal hygiene habit were risk factors for HPV infection in menopausal women(P<0.05).Conclusion The HPV infection rate of menopausal women is at a high level,and is affected by many factors.It is necessary to increase the publicity of HPV related knowledge for menopausal women.
