高度近视并发性白内障植入三焦点人工晶状体的临床疗效

摘　　要：目的观察高度近视并发性白内障患者植入AT LISA tri 839 MP三焦点人工晶状体术后视力,视觉质量和满意度。方法回顾性病例研究。选取我院高度近视并发性白内障患者30例(46只眼),所有术眼均行飞秒激光辅助白内障超声乳化吸除+囊袋张力环+三焦点人工晶状体植入术。记录术前、术后1 d、1周、1个月、3个月、6个月未矫正远视力(UDVA)、未矫正中视力(UIVA)、未矫正近视力(UNVA),以及术后6个月离焦曲线、对比敏感度,并进行满意度,脱镜率及VF-14-CN视功能量表的问卷调查。结果术后1 d、1周、1个月、3个月、6个月的UDVA、UIVA、UNVA均较术前提高,差异有统计学意义(P<0.01);术后6个月,离焦曲线0~-3.0 D范围内,视力均>0.2 logMAR,且较为平缓;术后6个月明环境和暗+眩光环境下不同空间频率(3、6、12、18 cpd)的对比敏感度均在同年龄段正常范围;术后满意度100%,远、中、近距离脱镜率分别为100%、100%、89.13%,VF-14-CN视功能量表得分(97.98±1.55)分,所有患者均愿意再次选择和向周围亲戚朋友推荐同款人工晶状体。结论AT LISA tri 839MP三焦点人工晶状体可为高度近视并发性白内障患者带来良好的远、中、近全程视力,且脱镜率及满意度高,视觉质量较好。Objective To observe the visual acuity,visual quality and satisfaction after implantation of trifocal intraocular lens AT LISA tri 839MP in patients with high myopic and cataract.Methods Retrospective case study.Thirty high myopic cataract patients(46 eyes)in our hospital were included.All eyes underwent femtosecond laser assisted phacoemulsification and capsular tension ring and trifocal intraocular lens implantation.Uncorrected distance visual acuity(UDVA),uncorrected intermediate visual acuity(UIVA),and uncorrected near visual acuity(UNVA)were recorded preoperatively,at 1 day,1 week,1 month,3 months,and 6 months postoperatively.Defocus curve,contrast sensitivity,spectacle independence,patient satisfaction and the Visual Function Index questionnaire(VF-14-CN)were assessed at 6 months postoperatively.Results UDVA,UIVA and UNVA were significantly improved at 1 day,1 week,1 month,3 months and 6 months postoperatively(P<0.05).The defocus curve at 6 months was stable,and showed 0.2 logMAR and better VA between 0 D and-3.0 D.Contrast sensitivity at each spatial frequencies(3,6,12,18 cycle per degree(CPD))under photopic and mesopic with or without glare were in normal range of the age-matched group.The overall patient satisfaction was 100%,and 100%,100%,89.13% patients achieved spectacle independence for far,intermediate and near distance respectively.The mean VF-14-CN score was(97.98±1.55).All patients would choose the implanted IOL again and recommend the surgery to family and friends.Conclusions AT LISA tri 839MP trifocal intraocular lens provided good distance,intermediate and nearvision,high levels of both spectacle independence and patient satisfaction,and good visual qualityin high myopic cataract patients.
