摘  要:目的探讨靶血管内注射重组人尿激酶原对急性心肌梗死经皮冠状动脉介入治疗(PCI)患者血流灌注及单核细胞趋化蛋白-1(MCP-1)、可溶性细胞间黏附分子-1(sICAM-1)水平的影响。方法以随机数字表法将2018年3月至2021年3月我院收治的106例急性心肌梗死患者分为对照组和观察组,各53例。对照组行PCI,观察组在对照组基础上于靶血管内注射重组人尿激酶原。比较两组的治疗效果。结果术后,观察组的心肌梗死溶栓治疗(TIMI)血流分级优于对照组(P<0.05)。术后,观察组的左前降支收缩期血流峰值、左前降支舒张期血流峰值、冠脉血流储备(CFVR)高于对照组(P<0.05)。术后,观察组的MCP-1、sICAM-1水平低于对照组(P<0.05)。术后,观察组的丙二醛(MDA)水平低于对照组,谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)水平高于对照组(P<0.05)。结论靶血管内注射重组人尿激酶原可改善急性心肌梗死PCI患者血流灌注,调节MCP-1、sICAM-1水平。Objective To investigate the effects of target vessel injection of recombinant human urokinase on blood perfusion and monocyte chemoattractant protein-1(MCP-1)and soluble intercellular adhesion molecule-1(sICAM-1)levels in patients with acute myocardial infarction undergoing percutaneous coronary intervention(PCI).Methods A total of 106 patients with acute myocardial infarction admitted in our hospital from March 2018 to March 2021 were were divided into control group and observation group by random number table method,with 53 cases in each group.The control group underwent PCI,and the observation group was given target vessel injection of recombinant human urokinase on the basis of the control group.The therapeutic effects of the two groups were compared.Results After operation,the thrombolysis in myocardial infarction(TIMI)blood flow grade of the observation group was better than that of the control group(P<0.05).After operation,left anterior descending artery systolic peak velocity,left anterior descending artery diastolic peak velocity and coronary flow velocity reserve(CFVR)in the observation group were higher than those in the control group(P<0.05).After operation,the levels of MCP-1 and sICAM-1 in the observation group were lower than those in the control group(P<0.05).After operation,the level of malonaldehyde(MDA)in the observation group was lower than that in the control group,and the levels of glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)were higher than those in the control group(P<0.05).Conclusion Target vessel injection of recombinant human urokinase can improve blood perfusion and regulate the levels of MCP-1 and sICAM-1 in patients with acute myocardial infarction undergoing PCI.
