OCTA评估硅油对孔源性视网膜脱离患者视乳头周围毛细血管密度及视网膜神经纤维层厚度的影响

摘　　要：目的利用光相干断层扫描血管成像(OCTA)评估硅油填充对孔源性视网膜脱离(RRD)患者视乳头周围毛细血管密度(VD)及视网膜神经纤维层(RNFL)厚度的影响。方法随机选择2019年1月至2022年8月因RRD在安徽医科大学第一附属医院行玻璃体切除联合硅油填充术的18例(18眼)患者为病例组,并选取患者另一眼为对照组(15眼)。利用OCTA在硅油取出术后1周内测量双眼视乳头周围VD及RNFL。结果病例组硅油填充时间为91~223 d,平均(149.27±36.51)d。病例组视乳头周围VD低于对照组[(45.21±5.57)%比(52.50±3.34)%],差异有统计学意义(P<0.05)。病例组视乳头周围鼻上、鼻下、下鼻、下颞、颞上、颞下、上颞及上鼻侧8个区域的VD也均低于对照组(P均<0.05)。但两组视乳头RNFL及周围8个区域厚度之间比较,差异均无统计学意义(P均>0.05)。VD及RNFL与硅油填充时间无相关性(P均>0.05)。结论RRD患者的硅油填充眼可出现视乳头周围VD降低,但RNFL无明显变化。Objective To evaluate the effect of silicone oil filling on peripapillary vessel density(VD)and retinal nerve fiber layer(RNFL)thickness in patients with rhegmatogenous retinal detachment(RRD)by using optical coherence tomography angiography(OCTA).Methods A total of 18 RRD patients who underwent vitrectomy combined with silicone oil filling in the First Affiliated Hospital of Anhui Medical University from January 2019 to August 2022 were randomly selected as the case group,and another eye was selected as the control group(15 cases).Results There were 10 males and 8 females in the enrolled 18 patients;their age ranged from 32 to 65 years,with an aver⁃age of(50.89±9.90)years old.The time of silicone filling range from 91 to 223 days,with an aver time of(149.27±36.51)days.The peripapil⁃lary VD in the case group(45.21±5.57)was lower than that in the control group[(45.21±5.57)vs(52.50±3.34),%],and the difference was sta⁃tistically significant(P<0.05).The VD of the eight peripapillary regions including nasal superior,nasal inferior,inferior nasal,inferior tempo,tempo inferior,tempo superior,superior tempo,and superior nasal in the case group was also lower than that in the control group(P all<0.05,respectively).There were no significant differences in the peripapillary RNFL and eight peripapillary areas between the two groups(P all>0.05).Then,no correlation was found among VD,RNFL and SO filling time(P all>0.05).Conclusions Peripapillary VD may decrease in SO filling eyes of RRD patients,but there is no significant change in peripapillary RNFL.
