经支气管镜肺活检对肺部周围型病变的诊断价值

摘　　要：目的:分析经支气管镜肺活检在肺部周围型病变中的诊断价值。方法:随机选取2020年1月—2022年6月邯郸市第四医院疑似肺部周围病变患者177例为研究对象,根据检查方式不同分为研究1组(n=47)、研究2组(n=42)、研究3组(n=39)及研究4组(n=49)。研究1组采用CT引导下经支气管镜肺活检,研究2组采用超声引导下经支气管镜肺活检,研究3组完成经支气管镜肺活检后实施支气管肺泡灌洗及纤支镜刷片,研究4组采用经皮肺穿刺活检。对比各组检测结果。结果:各组组织样本获取成功率比较,差异无统计学意义(P>0.05);研究3组肺活检病理结果与最终诊断结果符合率高于研究1组、研究2组及研究4组,差异具有统计学意义(P<0.05);经支气管镜肺活检组并发症总发生率低于经皮肺穿刺活检组,差异具有统计学意义(P=0.001)。结论:经支气管镜肺活检在肺部周围型病变中的诊断价值高于经皮肺穿刺活检,配合支气管肺泡灌洗及纤支镜刷片可进一步提高确诊率。Objective:to analyze the diagnostic value of bronchoscopic lung biopsy in peripheral pulmonary lesions.Methods:From January 2020 to June 2022,one hundred and seventy-seven patients with suspected pulmonary peripheral lesions in the Handan Fourth Hospital were randomly selected as study subjects,and were divided into study group 1(n=47),study group 2(n=42),study group 3(n=39)and study group 4(n=49)according to different examination methods.The study group 1 received CT-guided bronchoscopic lung biopsy,the study group 2 received ultrasound-guided bronchoscopic lung biopsy,the study group 3 received bronchoalveolar lavage and bronchoscopic brushing after bronchoscopic lung biopsy,and the study group 4 received percutaneous lung biopsy.Compare the test results of each group.Results:There was no significant difference in the success rate of tissue sample acquisition between each group(P>0.05).The coincidence rate of pathological results of lung biopsy and final diagnosis in study group 3 was higher than that in study group 1,study group 2 and study group 4,and the difference was statistically significant(P<0.05).The total incidence of complications in bronchoscopic lung biopsy group was lower than that in percutaneous lung biopsy group,and the difference was statistically significant(P=0.001).Conclusion:The diagnostic value of bronchoscopic lung biopsy in peripheral pulmonary lesions is higher than that of percutaneous lung biopsy.Combined with bronchoalveolar lavage and bronchoscopic brushing can further improve the diagnostic rate.
