正颌外科术对骨性Ⅲ类错[牙合]患者髁突轴角与前后向位置的影响

摘　　要：目的:探讨正颌外科术对骨性Ⅲ类错[牙合]患者髁突轴角与前后向位置的影响。方法:选取2020年6月~2021年12月就诊于我院口腔门诊接受正畸-正颌联合治疗的患者32例。比较手术前后的髁突轴角指标以及髁突前后位置情况。结果:T1与T0比较,轴向髁突轴角与冠状髁突轴角测量值差异均无统计学意义(P>0.05),矢状髁突轴角测量值增大(P<0.05)。与T0比较,T1右侧髁突位置中心位、后位及对应的Pullinger值测量值增大(P<0.05),而左侧髁突位置前位及对应的Pullinger值测量值增大(P<0.05),中心位测量值减小(P<0.05)。结论:骨性Ⅲ类错[牙合]畸形患者正颌手术前后髁轴在矢状平面上有明显差异,右侧髁突相对于关节窝后移,而左侧髁突则相对于关节窝前移。Objective:To investigate the effect of orthognathic surgery on the condylar axis angle and anterior-posterior position of patients with skeletal Class III malocclusion.Methods:A total of 32 patients who received orthodontic-orthognathic treatment in the dental clinic of our hospital from June 2020 to December 2021 were selected.The measured values of condylar axis angle indexes and the distribution of the anterior and posterior positions of the condyle before and after the operation were compared.Results:Compared with T0,there was no statistical significance between the axial condyle angle and the coronal condyle angle at T1(P>0.05),and the sagittal condyle angle measurement value increased(P<0.05).Compared with T0,the central position,posterior position and the corresponding Pullinger value of the right condyle position at T1 increased(P<0.05),while the anterior position of the left condyle position and the corresponding Pullinger value were increased(P<0.05),and the central measurement value decreased(P<0.05).Conclusion:In patients with skeletal Class III malocclusion,the condyle axis was also significantly different in the sagittal plane.The right condyle is posteriorly displaced relative to the glenoid,while the left condyle is anteriorly displaced relative to the glenoid.
