TFF3和HER2在食管胃交界部腺癌中的表达及临床意义

摘　　要：目的探讨三叶因子3(TFF3)及人表皮生长因子受体2(HER2)在食管胃交界部腺癌(AEG)组织中的表达及临床意义。方法选取140例行手术切除的AEG患者,采用免疫组化法检测AEG组织中TFF3的表达情况,免疫组化法及免疫荧光法检测HER2的表达情况,分析二者表达的差异及其与临床病理特征和预后的关系。结果在AEG组织中,TFF3的阳性表达率为51.43%,HER2的阳性表达率为15.00%。TFF3阳性多见于Laurén分型为弥漫型、出现淋巴结转移的患者。HER2阳性多见于Laurén分型为肠型的患者。HER2阴性患者TFF3阳性表达率高于阳性患者(χ^(2)=5.167,P=0.023)。Kaplan-Meier生存曲线分析显示,TFF3表达会影响AEG患者生存率,TFF3阳性的AEG患者生存率明显低于TFF3阴性患者。Cox回归分析结果显示,淋巴结转移、TNM分期较晚及TFF3阳性表达是AEG独立预后的危险因素(P<0.05)。结论TFF3阳性提示患者预后差,是影响AEG预后的独立危险因素;HER2的表达不影响AEG的预后。TFF3及HER2是AEG发生发展的重要因子,二者可能在AEG的发生发展中相互作用,是否可以作为治疗靶标有待进一步研究。Objective To investigate the expression and clinical significance of trefoil factor-3(TFF3)and human epidermal growth factor receptor 2(HER2)in adenocarcinoma of esophagogastric junction(AEG)tissue.Methods A total of 140 AEG patients who under-went surgical resection were selected,and the expression of TFF3 in AEG tissues was detected by immunohistochemistry,and the expression of HER2 was detected by immunohistochemistry and immunofluorescence.The differences in expression between the two and the relation-ship with clinicopathological features and prognosis were analyzed.Results In AEG tissues,the positive expression rate of TFF3 was 51.43%,and the positive expression rate of HER2 was 15.00%.Positive TFF3 is more common in patients with diffuse Laurén classifica-tion and lymph node metastasis.Positive HER2 is more common in patients with Laurén's intestinal type.The positive expression rate of TFF3 in HER2 negative patients was higher than that in HER2 positive patients(χ^(2)=5.167,P=0.023).Kaplan-Meier survival curve analysis showed that TFF3 expression can affect the survival rate of AEG patients,and the survival rate of AEG patients with positive TFF3 was significantly lower than that of TFF3 negative patients.Cox regression analysis showed that lymph node metastasis,late TNM staging,and TFF3 positive expression were the independent prognostic risk factors for AEG(P<0.05).Conclusion Positive TFF3 indicates poor prog-nosis in patients and is an independent risk factor affecting the prognosis of AEG.The expression of HER2 does not affect the prognosis of AEG.TFF3 and HER2 are important factors in the occurrence and development of AEG,and they may interact with each other in the occur-rence and development of AEG.Further research is needed to determine whether they can be used as therapeutic targets.
