双[牙合]垫矫治器对骨性Ⅱ类错[牙合]畸形软硬组织的改善作用

摘　　要：目的:探讨双[牙合]垫矫治器对骨性Ⅱ类错[牙合]畸形软硬组织的改善作用。方法:选取2018年10月-2021年5月笔者医院收治的102例骨性Ⅱ类错[牙合]畸形患儿纳入研究,给予患儿双[牙合]垫矫治器治疗。矫治周期为12个月,分别于矫治前后拍摄X线头颅侧位片,由专业人员进行测量、描图。对患儿矫治前后软硬组织形态进行比较,采用Ricketts美容线对矫治合格率进行评估。结果:矫治后,SNB、U1-NA等硬组织指标大于矫治前,ANB、L1-NB、U1-NA、L1-NB均小于矫治前(P<0.05);矫治后,FHN’Pm、上唇沟深度、Sn-H线距、Pog-Pm’等软组织指标均大于矫治前,H角小于矫治前(P<0.05);其余软硬组织测量项目比较差异无统计学意义(P>0.05);矫治后,102例患儿中有97例达到合格及以上标准,矫治合格率为95.09%;95例患儿对矫治效果满意,满意度为93.14%。结论:采用双[牙合]垫矫治器矫治骨性Ⅱ类错[牙合]畸形患儿可取得明显疗效,有效减少切牙突度及覆盖情况,在骨性Ⅱ类磨牙关系以及面部软硬组织形态方面具有较好的改善效果。Objective To investigate the efiect of Twin-block appliance on hard and soft tissues of skeletal classⅡmalocclusion.Methods A total of 102 patients with skeletal classⅡmalocclusion from October 2018 to May 2021 in the author's hospital hospital were enrolled in the study.They were treated with Twin-block appliance.The period of orthodontic treatment was 12 months.Lateral cephalograms were taken before and after orthodontic treatment,and measured and traced by professionals.The morphology of soft and hard tissues was compared before and after treatment,and the eligible rate was evaluated by Ricketts line.Results After treatment,the values of SNB,U1-NA were higher than those before treatment,while the values of ANB,L1-NB,U1-NA and L1-NB were lower than those before treatment(P<0.05).After treatment,FH-N,Pm,depth of upper lip sulcus,Sn-H line distance,Pog-Pm were all larger than those before treatment,but H angle was smaller than those before treatment(P<0.05).There was no significant difference in other soft and hard tissue measurement items(P>0.05).97 of the 102 patients treated with Twin-block appliance reached the qualified standard or above,and the qualified rate was 95.09%.95 patients were satisfied with the treatment,the satisfaction rate was 93.14%.Conclusion Twinblock appliance can efiectively reduce the protrusion and overjet of incisors,and improve the relationship of skeletal classⅡmolars and the morphology of facial soft and hard tissues in skeletal classⅡmalocclusion.
