摘　　要：目的 观察1例B型预激综合征发生阵发性室上性心动过速患者应用磁导航系统行右侧双旁道导管射频消融术的临床效果及安全性。方法 1例B型预激综合征发生室上性心动过速患者,男,39岁,应用磁导航系统行右侧双旁道导管射频消融术,先行希氏束旁旁道射频消融,再行右侧游离壁旁道射频消融。记录手术情况,随访12个月观察心动过速复发情况。结果 术中导管定位精准,应用长鞘辅助贴靠稳定,手术时间1 h, X线曝光时间30 s,手术过程顺利,未发生房室传导阻滞。随访至2023年1月,患者12导联体表心电图示无明显异常,心动过速未再发作。结论 应用磁导航系统行右侧双旁道导管射频消融术治疗B型预激综合征阵发性室上性心动过速,术中导管定位精准,贴靠稳定,X线曝光时间短,效果良好。Objective To observe the clinical efficacy and safety of magnetic navigation system guided catheter ablation of right-sided double accessory pathways in a patient with type B pre-excitation syndrome.Methods One male patient aged 39 years was diagnosed with type B pre-excitation syndrome and received magnetic navigation system guided catheter ablation of para-Hisian accessory pathway followed by catheter ablation of a right lateral accessory pathway.The operation outcome was observed and the patient was followed up for 12 months to record the recurrence of tachycardia.Results Under the guidance of the magnetic navigation system,the catheter with the aid of a long sheath achieved a good stability.The operation lasted for 1 h with 30 s of intermittent fluoroscopy.The procedure went smoothly,with no atrial ventricular block.This patient was followed up till January,2023,showing no tachycardia recurrence or abnormal 12-lead electrocardiogram.Conclusion During catheter ablation of right-sided double accessory pathways in treating type B pre-excitation syndrome,the magnetic navigation system enables precise manipulation and stable positioning of catheter with short fluoroscopy exposure time and good outcomes.
