血清CA125、SMRP、B7-H4联合检测用于卵巢癌诊断的敏感度与特异度分析

摘　　要：目的:研究血清糖类抗原125(CA125)、可溶性间皮素相关蛋白(SMRP)、T细胞共刺激分子B7(B7-H4)联合检测用于卵巢癌诊断的敏感度与特异度。方法:选取2022年1月—2023年1月甘肃省康复中心医院收治的81例卵巢癌患者作为卵巢癌组,另选取同时期80例卵巢良性病变患者作为卵巢良性病变组。检测并对比两组血清CA125、SMRP、B7-H4水平,以受试者工作特征(ROC)曲线分析血清CA125、SMRP、B7-H4联合检测诊断卵巢癌的效能。此外,分析不同TNM分期、淋巴结转移情况卵巢癌患者血清CA125、SMRP、B7-H4水平的差异。结果:卵巢癌组血清CA125、SMRP、B7-H4水平分别为(44.71±10.30)U/mL、(3.46±0.57)nmol/L、(277.19±24.68)μg/L,均高于卵巢良性病变组的(25.29±6.32)U/mL、(0.90±0.20)nmol/L、(16.87±2.59)μg/L,差异有统计学意义(P<0.05)。经ROC曲线分析证实,血清CA125、SMRP、B7-H4联合检测诊断卵巢癌的曲线下面积为0.884,高于三项指标单独检测,此时敏感度为87.07%、特异度为86.33%。TNM分期为Ⅲ期、Ⅳ期卵巢癌患者血清CA125、SMRP、B7-H4水平均高于Ⅰ期、Ⅱ期患者,差异有统计学意义(P<0.05)。淋巴结转移卵巢癌患者血清CA125、SMRP、B7-H4水平均高于无淋巴结转移患者,差异有统计学意义(P<0.05)。结论:CA125、SMRP、B7-H4在卵巢癌患者血清中均存在异常高表达,联合检测诊断卵巢癌具有较高的敏感度与特异度,且能有效反映病情严重程度。Objective:To investigate the sensitivity and specificity of serum carbohydrate antigen 125(CA125),soluble mesothelin-associated protein(SMRP)and T cells co-stimulate the molecule B7(B7-H4)combined detection in the diagnosis of ovarian cancer.Method:A total of 81 patients with ovarian cancer who admitted to the Gansu Provincial Rehabilitation Center Hospital from January 2022 to January 2023 were selected as the ovarian cancer group,another 80 patients with benign ovarian lesions in the same period were selected as the benign ovarian lesions group.Serum CA125,SMRP and B7-H4 levels were detected and compared between two groups.The efficacy of combined detection of serum CA125,SMRP and B7-H4 in the diagnosis of ovarian cancer was analyzed by receiver operating characteristic(ROC)curve.In addition,the differences of serum CA125,SMRP and B7-H4 levels in ovarian cancer patients with different TNM stages and lymph node metastasis were analyzed.Result:The serum levels of CA125,SMRP and B7-H4 in the ovarian cancer group were(44.71±10.30)U/mL,(3.46±0.57)nmol/L and(277.19±24.68)μg/L,respectively,they were higher than(25.29±6.32)U/mL,(0.90±0.20)nmol/L,(16.87±2.59)μg/L in the benign ovarian disease group,and the differences were statistically significant(P<0.05).ROC curve analysis confirmed that the area under the curve of the combined detection of serum CA125,SMRP and B7-H4 was 0.884,which was higher than those of three indexes alone,at this time,the sensitivity was 87.07%and the specificity was 86.33%.The levels of serum CA125,SMRP and B7-H4 in ovarian cancer patients with TNM stageⅢ,stageⅣwere higher than those in patients with stageⅠ,stageⅡ,and the differences were statistically significant(P<0.05).The serum levels of CA125,SMRP and B7-H4 in patients with lymph node metastasis were higher than those in patients without lymph node metastasis,and the differences were statistically significant(P<0.05).Conclusion:CA125,SMRP and B7-H4 are all abnormally high expressed in the serum of ovarian cancer patients.Com
