不同胸腔镜手术切除方式治疗早期肺癌肺部小结节患者的临床效果

摘　　要：目的:对比不同胸腔镜手术切除方式治疗早期肺癌肺部小结节患者的临床效果。方法:回顾性选取2019年1月—2020年12月于常熟市中医院胸心外科治疗的119例早期肺癌肺部小结节患者临床资料。根据手术不同切除方式将其分为肺楔形切除组(n=40)、肺段切除组(n=39)、肺叶切除组(n=40)。三组均给予胸腔镜手术。肺叶切除组给予肺叶切除胸腔镜手术,肺段切除组给予肺段切除胸腔镜手术,肺楔形切除组给予肺楔形切除胸腔镜手术。比较三组围手术期指标、并发症、复发及死亡情况。结果:肺段切除组手术时间、术后住院时间均短于肺叶切除组,引流总量少于肺叶切除组,视觉模拟评分法(VAS)评分低于肺叶切除组;肺楔形切除组手术时间短于肺叶切除组和肺段切除组,总费用和引流总量均少于肺叶切除组和肺段切除组,置管时间和术后住院时间均短于肺叶切除组,VAS评分低于肺叶切除组和肺段切除组,差异有统计学意义(P<0.05)。三组术后并发症发生率比较,差异无统计学意义(P>0.05)。三组术后1年无死亡及复发情况。结论:在早期肺癌肺部小结节患者的治疗中,肺楔形切除和肺段切除这两种术式创伤小,有助于患者术后康复,以肺楔形切除术更优。相较于前两种,肺叶切除术创伤大,术后住院时间长,三种手术方式并发症无明显差异。Objective:To compare the clinical effect of thoracoscopic surgery with different resection methods in the treatment of patients with early lung cancer small pulmonary nodule.Method:The clinical data of 119 patients with early lung cancer small pulmonary nodules treated in the Thoracic and Cardiac Surgery Department of Changshu Hospital of Traditional Chinese Medicine from January 2019 to December 2020 were retrospectively selected.According to different surgical resection methods,the patients were divided into pulmonary wedge resection group(n=40),pulmonary segmental resection group(n=39)and pulmonary lobectomy group(n=40).Thoracoscopic surgery was performed in all three groups.Pulmonary lobectomy group was given pulmonary lobectomy thoracoscopic surgery,pulmonary segmental resection group was given pulmonary segmental resection thoracoscopic surgery,and pulmonary wedge resection group was given pulmonary wedge resection thoracoscopic surgery.The perioperative indexes,complications,recurrence and death condition of the three groups were compared.Result:The operative time and postoperative hospitalization time of pulmonary segmental resection group were shorter than those of pulmonary lobectomy group,the total drainage volume was less than that of pulmonary lobectomy group,and the visual analogue scale(VAS)score was lower than that of pulmonary lobectomy group,the operative time of the pulmonary wedge resection group was shorter than that of the pulmonary lobectomy group and the pulmonary segmental resection group,the total cost and total drainage volume were less than those of the pulmonary lobectomy group and the pulmonary segmental resection group,the catheter placement time and postoperative hospital stay were shorter than those of the pulmonary lobectomy group,and the VAS score was lower than that of the pulmonary lobectomy group and the pulmonary segmental resection group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications
