戟天健脑方联合尼麦角林治疗老年性痴呆患者的效果研究

摘　　要：目的:探讨戟天健脑方联合尼麦角林对老年性痴呆患者血清脑源性神经营养因子(BDNF)、脂蛋白相关磷脂酶A2(Lp-PLA2)及炎性因子水平的影响。方法:选入我院2018年12月—2021年12月收治的老年性痴呆患者150例,根据治疗方法不同分为对照组和联合组,每组75例,其中对照组予以尼麦角林口服治疗,联合组在对照组基础上联合戟天健脑方治疗。比较两组治疗效果、不良反应、血清BDNF、Lp-PLA2及炎性因子水平变化。结果:联合组治疗总有效率为84.00%,高于对照组的66.67%(P<0.05);联合组MMSE和ADL量表得分高于对照组(P<0.05);联合组血清BDNF高于对照组,Lp-PLA2、IL-6、IL-1β、TNF-α和CRP低于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:戟天健脑方联合尼麦角林治疗老年性痴呆疗效肯定,可增加血清BDNF水平、降低Lp-PLA2及炎性因子水平,改善患者认知功能及生活能力,且安全性良好。Objective:To investigate the effect of Jitian Jiannao Decoction combined with Nicergoline on serum levels of brain-derived neurotrophic factor(BDNF),lipoprotein-related phospholipase A2(Lp-PLA2)and inflammatory factors in patients with senile dementia.Methods:A total of 150 patients with senile dementia admitted to our hospital from December 2018 to December 2021 were selected and divided into control group and combined group according to different treatment methods,with 75 cases in each group.The control group was treated with oral administration of nicergoline,and the combined group was treated with Jitian Jiannao decoction on the basis of the control group.The therapeutic effect,adverse reactions,serum BDNF,Lp-PLA2 and inflammatory factors were compared between the two groups.Results:The total effective rate of the combined group was 84.00%,which was higher than 66.67%of the control group(P<0.05);the MMSE and ADL scale scores in the combined group were higher than those in the control group(P<0.05);the serum BDNF in the combined group was higher than that in the control group,Lp-PLA2,IL-6,IL-1β,TNF-αand CRP were lower than those in the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Jitian Jiannao decoction combined with nicergoline is effective in the treatment of senile dementia,which can increase the serum BDNF level,reduce the levels of Lp-PLA2 and inflammatory factors,improve the cognitive function and life ability of patients,and has good safety.
