摘　　要：目的:研究液体活检在小细胞肺癌个体化疗中临床应用价值。方法:选取深圳市宝安区人民医院从2017年1月-2019年4月40例小细胞肺癌患者。对所有受试者均开展液体活检,并根据活检结果分作游离循环肿瘤细胞(CTCs)阳性组与CTCs阴性组。分析CTCs与患者临床特征以及治疗效果的关系,比较两组生物标志物[包括经元特异性烯醇化酶(NSE)、胃泌素释放肽前体(PrpCRP)以及人中心体蛋白350(CEP350)]的水平,并分析CTCs与小细胞肺癌患者生物标志物的相关性。结果:CTCs阳性组患者临床分期Ⅲ期-Ⅳ期人数占比为56.00%,高于CTCs阴性组的20.00%,差异有统计学意义(P<0.05)。CTCs阳性组患者疾病控制率低于CTCs阴性组,差异有统计学意义(P<0.05)。CTCs阳性组血清NSE、PrpCRP以及CEP350水平均高于CTCs阴性组,差异有统计学意义(P<0.05)。经Spearman相关性分析发现,小细胞肺癌患者CTCs阳性与血清NSE、PrpCRP以及CEP350水平均呈正相关关系(P<0.05)。结论:液体活检在小细胞肺癌个体化疗中临床应用价值较高,可能具有较好的应用前景。Objective To study the clinical value of liquid biopsy in individual chemotherapy of small cell lung cancer.Method 40 patients with small cell lung cancer treated in the hospital from January 2017 to April 2019 were selected.Liquid biopsy was performed on all subjects,and they were divided into CTCs positive group and CTCs negative group according to the biopsy results.The relationship between CTCs and clinical characteristics and therapeutic effect of patients was analyzed.The levels of biomarkers[including transmembrane specific enolase(NSE),gastrin releasing peptide precursor(PrpCRP)and human centrosome protein 350(CEP350)]were compared between the two groups,and the correlation between CTCs and biomarkersin patients with small cell lung cancer was analyzed.Results The proportion of patients with clinical stagesⅢ-Ⅳin CTC positive group was 56.00%,which was higher than 20.00%in CTCs negative group(P<0.05).The disease control rate of CTCs positive group was lower than that of CTCs negative group(P<0.05).The levels of serum NSE,PrpCRP and CEP350 in CTCs positive group were higher than those in CTCs negative group(P<0.05).Spearman correlation analysis showed that the positive expression of CTCs in patients with small cell lung cancer was positively correlated with the levels of serum NSE,PrpCRP and CEP350(P<0.05).Conclusion Liquid biopsy has high clinical value in individual chemotherapy of small cell lung cancer,and may have a good application prospect.
