舌骨上间室外科切除对晚期舌癌的救治研究

摘　　要：目的 通过对晚期舌癌行舌骨上间室外科切除患者生活质量毁损及预后情况的简单评估,评价晚期舌癌超扩大切除的临床意义。方法 收集我科自2019年5月至2021年4月收治的17例接受舌颌颈联合根治+舌原发灶间室切除的晚期舌鳞癌患者,对入组患者的术中情况、术后并发症和口腔功能等进行随访评价。结果 17例患者原发灶切除顺利,在手术耗时及术中出血无明显增加,术中切缘阳性率11.8%,随访期内暂无患者因舌癌死亡,吞咽、咀嚼及言语的Likert评分为3个月(2.471, 3.176, 3.471),6个月(3.706, 3.941, 3.824)以及1年(4.094, 4.353, 4.118)。结论 基于舌功能及结构的特异性,舌骨上间室切除能有效降低局晚期舌癌原发灶切除后的局部复发率,口腔功能的损失无明显加大。Objective To evaluate the clinical significance of super extended resection of advanced tongue cancer.Methods A total of 17 patients with advanced squamous cell carcinoma of tongue were collected.The intra-operative conditions,postoperative complications and oral function of the enrolled patients were followed up and evaluated.Results The primary focus of 17 patients was removed smoothly without significant increase in operation time and intraoperative bleeding.The pathological positive rate of intraoperative margin was 11.8%.During the follow-up period,no patient died of tongue cancer.The Likert scores of swallowing,chewing and speech were 3 months(2.471,3.176,3.471),6 months(3.706,3.941,3.824)and 1 year(4.094,4.353,4.118).Conclusions Based on the specificity of tongue function and structure,the application of supra hyoid compartmental resection in the surgical treatment of advanced tongue cancer can effectively reduce the local recurrence,and the loss of oral function is not significantly increased.
