珍龙醒脑胶囊治疗脑梗死恢复早期肢体运动障碍的临床研究

摘　　要：目的:探索珍龙醒脑胶囊治疗脑梗死恢复早期肢体运动障碍的临床疗效和作用机制。方法:对29例脑梗死恢复早期肢体运动障碍的患者,给予4周的藏药珍龙醒脑胶囊口服治疗。治疗4周后,观察患者《中风病证候要素评价量表》、Fugl-Meyer运动功能评定量表(Fugl-Meyer Assessment,FMA)和美国国立卫生研究院卒中量表(National Institutes of Health Stroke Scale,NIHSS)评分的改变及不良反应发生情况,并在治疗前后对患者进行磁共振扩散张量成像,采用基于纤维束示踪的空间统计方法获取治疗前后各向异性分数值差异的脑区进行分析。结果:治疗后患者的内火、痰湿、血瘀证候的积分较治疗前降低(P<0.05);FMA总分和NIHSS总分较治疗前改善(P<0.05);胼胝体体部、胼胝体压部、胼胝体毯、丘脑后辐射、放射冠、左侧上纵束、左侧外囊、右侧内囊前肢等脑区的各向异性分数值较治疗前升高(P<0.05)。治疗期间,受试者无不良反应发生。结论:珍龙醒脑胶囊治疗脑梗死恢复早期肢体运动障碍的患者,用药4周是有效且安全的。珍龙醒脑胶囊能有效改善脑梗死恢复期患者的肢体运动功能障碍,其作用机制可能与胼胝体、放射冠等脑区受损白质纤维束的修复与重组相关。Objective:To explore the clinical efficacy and action mechanism of Zhenlong Xingnao capsules(珍龙醒脑胶囊)in the treatment of limb movement disorders in the early recovery period of patients with cerebral infarction.Methods:A total of 29 enrolled patients were administered Zhenlong Xingnao capsules for four weeks.After the completion of treatment,the changes in the scores of Stoke Syndrome Factor Evaluation Scale,Fugl-Meyer motor function scale,and National Institute of Health Stroke Scale(NIHSS),and the occurrence of adverse effects were observed.The diffusion tensor imaging(DTI)and tract-based spatial statistics(TBSS)methods were applied to analyze the changes in white matter fiber tracts before and after treatment.Results:After the treatment,the scores of internal fire,phlegm-dampness,and blood stasis symptoms were decreased compared with those before the treatment(P<0.05).Besides,the scores of the FMA and NIHSS were ameliorated compared with those before the treatment(P<0.05).Fractional anisotropy values in some brain regions were higher than those before the treatment,such as corpus callosum,posterior thalamic radiation,radial corona,left superior longitudinal fasciculus,left external capsule,and right anterior limb of the internal capsule(P<0.05).No adverse events happened during the treatment.Conclusion:The four-week treatment of the Zhenlong Xingnao capsules is effective and safe for cerebral infarction patients with limb movement disorders in the early recovery period.Zhenlong Xingnao capsules can effectively improve the motor functions of patients.The mechanism might be related to the repair and reorganization of damaged white matter fiber bundles in the corpus callosum,radial corona,and other brain regions.
