复方斑蝥胶囊联合持续热灌注化疗对进展期胆管癌的疗效研究

摘　　要：目的探讨复方斑蝥胶囊联合持续热灌注化疗(CHPPC)用于进展期胆管癌的治疗效果。方法选取进展期胆管癌患者100例,采用随机对照表法分为观察组与对照组,各50例。对照组于胆管癌姑息切除术后给予CHPPC治疗,观察组采用复方斑蝥胶囊联合CHPPC治疗,共治疗6个疗程。评价2组治疗后临床疗效,观察2组治疗前后KPS评分变化,全自动生化分析仪检测患者肝功能指标(TB、AST、ALT、γ-GT)水平变化,ELISA实验检测患者血清中肿瘤标志物(CEA、CA199、CA125)水平变化,记录患者不良反应发生情况。结果2组治疗前各指标差异均无统计学意义(P>0.05),治疗后,观察组临床有效率较对照组显著提高,2组TB、AST、ALT、γ-GT肝功能及CEA、CA19-9、CA125肿瘤标志物水平均明显下降(P<0.05),生活质量较治疗前明显改善(P<0.05);观察组TB、AST、ALT、γ-GT及CEA、CA19-9、CA125水平较对照组均显著降低(P<0.05),生活质量改善率较对照组明显提高(P<0.05),不良反应发生率明显低于对照组(P<0.05)。结论复方斑蝥胶囊联合CHPPC治疗能够进一步改善进展期胆管癌患者的临床症状,改善患者肝功能和生活质量,疗效更好,能够减轻不良反应,安全性较好。Objective To investigate the therapeutic effect of compound cantharides capsules combined with continuous hyperthermic perfusion chemotherapy(CHPPC)in the treatment of advanced cholangiocarcinoma.Methods One hundred patients with the disease were randomly divided into an observation group and a control group,with 50 cases in each group.The control group was treated with CHPPC after the palliative resection of cholangiocarcinoma,while the observation group was treated with compound cantharidin capsules combined with CHPPC,both for 6 courses.The clinical efficacy of the two groups after treatment was evaluated,the changes in the KPS score were observed before and after treatment,the changes of liver function indexes(TB,AST,ALTγ-GT)were detected by automatic biochemical analyzer,and the changes of tumor markers(CEA,CA199,CA125)in the patients’serum were detected by ELISA assay,and the occurrence of adverse reactions was recorded.Results There was no significant difference in all the indicators between the two groups before treatment(P>0.05).After treatment,the clinical effective rate of the observation group was significantly improved compared to that of the control group,and the levels of TB,AST,ALT,γ-GT in the liver function and CEA,CA19-9,CA125 in the tumor markers were decreased significantly in the two groups(P<0.05),and the quality of life was improved significantly(P<0.05).The levels of TB,AST,ALTγ-GT and CEA,CA19-9,CA125 were significantly lower than those in the control group(P<0.05).The life quality improvement rate was significantly higher than that in the control group(P<0.05),and the incidence of adverse reactions was significantly lower than that in the control group(P<0.05).Conclusion Compound cantharidin capsules combined with CHPPC can further improve the clinical symptoms,the liver function and the quality of life in patients with advanced cholangiocarcinoma,which can achieve a better efficacy,reduce adverse reactions and have a better safety.
