腭部骨折24例回顾分析

摘　　要：目的:总结我院收治的24例腭部骨折的诊疗经验,以期为腭部骨折治疗方法的选择、降低术后并发症提供参考。方法:回顾性分析我院收治的24例腭部骨折的患者资料,包括年龄、性别、症状、骨折类型、治疗方法和术后并发症。结果:本组24例腭部骨折患者中,3例患者接受保守治疗,其余接受手术治疗的21例患者中,HendricksonⅡ型行腭间牵引复位,Ⅲ型行腭间牵引复位+颊侧断端的钛板固定和颅颌面其他部位的骨折治疗。接受手术治疗的患者在术后6个月的随访中发现骨折断端愈合良好,无明显不良并发症。结论:临床上对于颅颌面部多发骨折的患者,容易遗漏腭部骨折的处理,优先进行腭部骨折的精准对合有助于恢复良好的咬合关系和颌面部其他部位骨折的处理。Objective:To summarize the experience in diagnosis and treatment of 24 cases of palatal fracture in order to provide reference for the selection of treatment methods of palatal fracture and reduce postoperative complications.Methods:The data of 24 patients with palatal fracture admitted to our hospital were analyzed retrospectively,including age,sex,symptoms,fracture types,treatment methods,and postoperative complications.Results:Of the 24 patients with palatal fracture in this group,3 patients received conservative treatment,and the other 21 patients received surgical treatment,in which Hendrickson typeⅡwere underwent palatal traction reduction and typeⅢwere underwent palatal traction reduction+titanium plate fixation at the buccal fracture end and fracture treatment at other parts of the craniomaxillofacial region.During the follow-up of 6 months after operation,the patients who received surgical treatment had the fracture ends healed well without obvious adverse complications.Conclusion:For patients with multiple craniomaxillofacial fractures,it is easy to omit the treatment of palatal fractures.Priority should be given to the accurate alignment of palatal fractures,which will help to restore good occlusal relationship and the treatment of fractures in other parts of the maxillofacial regions.
