气压泵治疗机改善乳腺癌改良根治术后皮下积液的有效性及安全性研究

摘　　要：目的探究气压泵治疗机改善乳腺癌改良根治术后皮下积液的有效性及安全性。方法选择2018年2月—2022年2月德州市立医院完成乳腺癌改良根治术后皮下积液患者64例作为研究对象,根据随机信封抽签原则将患者分为气压泵组与对照组,每组32例。对照组给予胸带加压包扎、肢体功能锻炼等常规方法进行治疗,气压泵组在对照组治疗的基础上给予气压泵治疗机治疗,两组均治疗观察14 d,记录两组的疗效优良率、皮下积液量、皮下引流量,判定上肢功能量表(DASH)评分与生活质量评分状况。结果治疗后,气压泵组疗效优良率高于对照组,但差异无统计学意义(P>0.05);气压泵组治疗后的皮下积液量、皮下引流量均低于对照组,差异有统计学意义(P<0.05)。两组DASH上肢功能评分高于治疗前,差异有统计学意义(P<0.05);气压泵组DASH上肢功能评分高于对照组,差异有统计学意义(P<0.05)。气压泵组生理状况、社会/家庭状况、情感状况、功能状况、附加关注状况等生命质量评分均高于对照组,差异有统计学意义(P<0.05)。结论气压泵治疗机能降低乳腺癌改良根治术后皮下积液患者的皮下积液量、皮下引流量,还可改善患者总体治疗效果,提高患者上肢功能与总体生命质量,具有很好的应用效果。Objective To investigate the efficacy and safety of pneumatic pump in improving subcutaneous effusion after modifi ed radical mastectomy for breast cancer.Methods From February 2018 to February 2022,64 cases of patients with postoperative subcutaneous effusion after modified radical surgery for breast cancer were selected as the study subjects.Accorded to the principle of random envelope drawing,the patients were divided into the pneumatic pump group of 32 cases and the control group of 32 cases.The control group were treated with conventional methods such as chest band compression bandage,limb function exercise,and the air pump group were treated with air pump therapy machine on the basis of the control group.Both groups were treated and observed for 14 days,and recorded the excellent and good rate of curative eff ect,subcutaneous fl uid accumulation and subcutaneous drainage fl ow of the two groups,and determine the score of the Disability of Arm Shoulder and Hand(DASH)and quality of life score.Results The excellent efficacy rate of the pneumatic pump group was higher than that in the control group,but there was no difference(P>0.05).The volume of subcutaneous fluid accumulation and subcutaneous drainage volume in the air pump group were signifi cantly lower than those in the control group(P<0.05).The DASH upper limb function score in the air pump group and the control group after treatment were signifi cantly higher than that before treatment(P<0.05),and the DASH upper limb function score in the air pump group after treatment were higher than that in the control group(P<0.05).The life quality scores of physiology,society/family,emotion,function and additional attention in the air pump group after treatment were signifi cantly higher than those in the control group(P<0.05).Conclusion The pneumatic pump therapy can reduce the subcutaneous fl uid volume and subcutaneous drainage volume of patients with subcutaneous fl uid after modifi ed radical mastectomy for breast cancer,improve the overall treatment eff ect o
