摘 要:目的利用新一代扫频源前节OCT研究不同时期皮质性白内障晶状体的偏心距离和倾斜度及其眼部生物学参数相关因素。方法横断面研究。选取2020年4月至2020年5月就诊于北京同仁医院眼科的健康右眼57例57眼为对照组;筛选同时期右眼诊断为皮质性白内障初发期患者55例55眼及皮质性白内障膨胀期的患者54例54眼为初发期组和膨胀期组。收集所有人员的一般资料,并获取其晶状体偏心距离和倾斜度、前房深度(ACD)、晶状体厚度(LT)、眼前节长度(ASL)、前房宽度(ACW)、晶状体拱高(LV)、α角参数。采用方差分析或Kruskal-Wallis检验分析3组晶状体偏心距离和倾斜度等参数的差异;采用Pearson或Spearman相关性检验分析晶状体偏心距离和倾斜度的相关因素。结果对照组、初发期组和膨胀期组晶状体偏心距离分别为(0.17±0.09)mm、0.14(0.08,0.18)mm、0.23(0.13,0.28)mm;晶状体倾斜度分别为(3.61±1.39)°、(3.94±1.32)°、(5.41±2.06)°,3组间比较,差异均有统计学意义(均为P<0.05)。晶状体的偏心距离与AL、ACD、α角均呈正相关;晶状体的倾斜度与AL、ACD均呈负相关,与年龄、LT、LV、α角均呈正相关(均为P<0.05)。结论皮质性白内障膨胀期晶状体可出现较大偏心距离和倾斜度,且多种眼部生物学参数均对晶状体的偏心距离和倾斜度发展变化相关。Objective To study the lens decentration and tilt as well as other ocular biological parameters in different stages of cortical cataracts using a new generation of swept-source anterior segment optical coherence tomography.Methods A cross-sectional study was carried out.Totally 57 patients(57 eyes)with healthy right eyes who visited the Ophthalmology Department of Beijing Tongren Hospital from April 2020 to May 2020 were selected as the control group;55 patients(55 eyes)diagnosed as early stage of cortical cataracts in the right eye and 54 patients(54 eyes)diagnosed as intumescent stage of cortical cataract in the right eye during the same period were selected as the early stage group and the intumescent stage group,respectively.The general information of all subjects was collected,and their lens decentration and tilt,anterior chamber depth(ACD),lens thickness(LT),anterior segment length(ASL),anterior chamber width(ACW),lens vault(LV)and angleαparameter were obtained.The analysis of variance or Kruskal-Wallis test was used to analyze the differences in parameters such as lens decentration and tilt among the three groups;Pearson or Spearman correlation test was used to analyze the factors related to lens decentration and tilt.Results The lens decentration of the control group,the early stage group and the intumescent stage group were(0.17±0.09)mm,0.14(0.08,0.18)mm and 0.23(0.13,0.28)mm,respectively;the lens tilt was(3.61±1.39)°,(3.94±1.32)°and(5.41±2.06)°,respectively,the differences among the three groups were statistically significant(both P<0.05).The decentration of the lens is positively correlated with axial length(AL),ACD and angleα;the tilt of the lens is negatively correlated with AL and ACD and positively correlated with age,LT,LV and angleα(all P<0.05).Conclusion In the intumescent stage of cortical cataract,the lens may have significant decentration and tilt,whose development is relevant to various ocular biological parameters.
