13例睾丸原发癌及其转移癌^(18)F-FDG PET/CT表现

摘　　要：目的观察睾丸原发癌及其转移癌^(18)F-FDG PET/CT表现。方法回顾性观察13例经病理证实的睾丸原发癌患者治疗前^(18)F-FDG PET/CT资料,分析原发癌及其转移癌影像学特点。结果共检出14个原发肿瘤,包括7例7个精原细胞瘤及6例7个非精原细胞肿瘤,平均最大径(3.44±1.75)cm;PET/CT均呈^(18)F-FDG高摄取,中位最大标准摄取值(SUV_(max))10.20(5.95,22.10),精原细胞瘤[9.10(4.70,14.50)]与非精原细胞肿瘤[12.20(6.30,23.10)]SUV_(max)差异无统计学意义(Z=-0.96,P=0.338)。睾丸原发肿瘤SUV_(max)与其最大径、Ki-67指数均无明显相关(P均>0.05)。1例(1/13,7.69%)左侧精索受侵,SUV_(max)8.80。13例中,11例(11/13,84.62%)存在淋巴结转移,以腹膜后淋巴结最常见,其次为纵隔及髂血管旁淋巴结转移,均表现为^(18)F-FDG高摄取,平均SUV_(max)15.65±7.97;8例(8/13,61.54%)发生血行转移,多表现为^(18)F-FDG高摄取,以肺及肝脏受累最多,平均SUV_(max)分别为12.66±8.55和16.42±8.81。结论睾丸原发癌均表现为^(18)F-FDG摄取增高,可侵犯精索并转移至全身淋巴结、肺及肝脏等;转移癌亦多^(18)F-FDG高摄取。Objective To observe^(18)F-FDG PET/CT manifestations of primary testicular cancer.Methods^(18)F-FDG PET/CT data of 13 patients with testicular cancer confirmed by pathology were reviewed,and the imaging findings were analyzed.Results A total of 14 primary testicular tumors were detected in 13 patients,including 7 seminomas in 7 cases and 7 nonseminomas in 6 cases,with the average maximum diameter of(3.44±1.75)cm,all with high uptake of^(18)F-FDG on PET/CT,and the media maximum standard uptake value(SUV_(max))was 10.20(5.95,22.10).No significant difference of SUV_(max)was found between seminomas(9.10[4.70,14.50])and nonseminomas(12.20[6.30,23.10])(Z=-0.96,P=0.338).No obvious correlation was observed between SUV_(max)and the maximum diameter nor Ki-67 index of lesions(both P>0.05).One primary tumor(1/13,7.69%)invaded the left spermatic cord with SUV_(max)of 8.80.Among 13 cases,11(11/13,84.62%)accompanied by lymph node metastases,most commonly in the retroperitoneum,followed by the mediastinum and iliac vessels.All lymph node metastases showed high uptake of^(18)F-FDG,with an average SUV_(max)of 15.65±7.97.Blood metastases were detected in 8 cases(8/13,61.54%),usually in the lungs and liver,with the average SUV_(max)of 12.66±8.55 and 16.42±8.81,respectively,most showed high uptake of^(18)F-FDG.Conclusion The primary lesions of testicular cancer had increased uptake of^(18)F-FDG,which could invade the spermatic cord and metastasize to lymph nodes,lungs and liver,most showed high uptake of^(18)F-FDG.
