无托槽隐形矫治器矫治安氏Ⅱ类错颌畸形伴中度牙周炎患者的效果

摘　　要：目的探讨无托槽隐形矫治器矫治安氏Ⅱ类错颌畸形伴中度牙周炎患者的效果。方法将我院2021年1月至2023年1月期间80例安氏Ⅱ类错颌畸形伴中度牙周炎患者分组,按照随机数字表法分为对照组40例,给予直丝弓矫治器矫治,观察组40例给予无托槽隐形矫治器矫治,观察矫治效果、矫治时间、口腔功能及牙周炎性水平。结果两组患者矫治效果各指标(牙齿排列、颊舌向倾斜度、咬合关系、邻牙边缘嵴高度、邻牙接触关系、咬合接触、覆盖情况与牙根平行度)对比无统计学意义(P>0.05);观察组患者矫治时间(倾斜牙、转为牙)均较对照组短(P<0.05);矫治后观察组患者美观度、咀嚼功能及舒适度评分均高于对照组(P<0.05);矫治后两组患者白细胞介素-1β(IL-1β)、C反应蛋白(CRP)、肿瘤坏死因子-α水平(TNF-α)水平较矫治前升高,但观察组低于对照组(P<0.05)。结论无托槽隐形矫治器应用于安氏Ⅱ类错颌畸形伴中度牙周炎患者可有效复原牙齿,缩短矫治时间,改善口腔功能,缓解牙周炎症。Objective To investigate the effect of non bracket invisible appliance in the treatment of Class II malocclusion with moderate Periodontal disease.Methods From January 2021 to January 2023,80 patients with Class II malocclusion with moderate Periodontal disease in our hospital were divided into control group(40 cases)and observation group(40 cases).Results showed that there was no statistically significant difference between the two groups of patients in terms of various indicators of orthodontic effect(tooth arrangement,buccal lingual inclination,occlusal relationship,adjacent tooth edge ridge height,adjacent tooth contact relationship,occlusal contact,coverage,and root parallelism)(P>0.05);The treatment time(tilted teeth,converted teeth)of the observation group patients was shorter than that of the control group(P<0.05);After correction,the aesthetic appearance,chewing function,and comfort scores of the observation group were higher than those of the control group(P<0.05);Interleukin-1 in two groups of patients after correctionβ(IL-1β)、C-reactive protein(CRP),tumor necrosis factor-αHorizontal(TNF-α)The level increased compared to before correction,but the observation group was lower than the control group(P<0.05).Conclusion The application of non bracket invisible appliance in the patients with Class II malocclusion with moderate Periodontal disease can effectively restore teeth,shorten the treatment time,improve oral function and alleviate periodontal inflammation.
