微种植支抗钉结合滑动杆治疗安氏Ⅱ类亚类错(牙合)畸形的疗效分析

摘  要:目的应用微种植支抗钉结合滑动杆治疗处于轻中度牙列拥挤安氏Ⅱ类亚类患者,探讨矫治前后疗效变化。方法对35例15~25岁轻中度牙列拥挤安氏Ⅱ类亚类患者采用单侧植入微种植体支抗钉结合滑动杆推磨牙向后,运用直丝弓矫治技术完成矫治。分析测量矫治前后模型及拍摄头颅侧位片评价其治疗有效性。结果治疗完成后所有患者远中侧磨牙由Ⅱ类关系调整为Ⅰ类关系,模型测量上上颌第一磨牙后移量平均增加5.15mm,牙弓前段、中段宽度平均增加分别为5.45mm、4.55mm,头影测量上上颌第一磨牙至Y轴距离平均减小3.3mm(牙合)平面顺时针旋转约2.72°,上颌磨牙轻度平均压低0.92mm,变化具有统计学意义(P<0.05),矫治效果良好。结论微种植支抗钉结合滑动杆能够有效远移上颌磨牙,可顺利完成对安氏Ⅱ类亚类轻中度牙列拥挤患者的矫治,疗效满意。Objective To use micro-implant anchorage screws combined with sliding rods to treat mild to moderate dentition crowding Angle's class Ⅱ subclass patients,and to explore the curative effect changes before and after treatment.Methods 35 patients aged 15~25 years with mild to moderate dentition crowding,Angle's Class Ⅱ subcategory were treated by unilateral implantation of micro-implant anchorage screws combined with sliding rods to push the molars backward,and straight wire arch correction technology was used.The model before and after treatment was analyzed and the lateral skull radiograph was taken to evaluate the treatment effectiveness.Results After the treatment,the distal molars of all patients were adjusted from class II relationship to class I relationship.The average maxillary first molar relocation increased by 5.15 mm,and the average width of the front and middle segments of the dental arch increased by 5.45 mm and 4.55 mm by model,respectively.,the average distance from the upper and maxillary first molars to the Y-axis was reduced by 3.3mm,and the clockwise rotation of the occlusal plane was about 2.72°,and the upper molars were mildly depressed by an average of 0.92mm by cephalometric measurement,and the changes were statistically significant(P<0.05),and the correction effect was good.Conclusion Micro-implant anchorage screws combined with sliding rod can effectively move the maxillary molars away,and can successfully complete the correction of patients with mild to moderate dental congestion of Angle's class II subclasses,and the curative effect is satisfactory.
