摘 要:目的基于倾向评分匹配法研究影像融合导航技术引导经皮穿刺活检在肺深部微小癌变检测中应用的价值。方法选取2018年4月至2021年4月河北省胸科医院收治的影像融合导航技术引导经皮穿刺肺活检术患者105例为观察组,选择同期本院常规CT引导下经皮穿刺肺活检术患者223例为对照组。分析两种方法对肺深部微小癌变的诊断准确性及患者并发症发生情况的差异。结果采用1∶1最邻近匹配法,倾向评分匹配法匹配后,观察组90例,对照组90例。与对照组比较,观察组操作时间、探针次数减少(P<0.05),针刺深度增加(P<0.05),气胸、少量出血并发症发生率、总并发症发生率降低(P<0.05)。与对照组比较,观察组诊断腺癌、鳞癌、小细胞癌的准确性均提高(P<0.05)。结论影像融合导航技术引导经皮穿刺活检对肺深部微小癌变的诊断具有较高的准确性,且具有操作时间短、辐射剂量小、并发症发生率低等特点,值得临床推广应用。Objective To evaluate the application value of image fusion navigation technology guided percutaneous biopsy in the detection of deep lung micro canceration based on propensity score matching method.Methods 105 patients with percutaneous lung biopsy guided by image fusion navigation technology admitted to Hebei chest hospital from April 2018 to April 2021 were selected as the observation group,and 223 patients with conventional CT guided percutaneous lung biopsy during the same period were selected as the control group.The difference between the two groups in the diagnosis of deep lung micro canceration and the incidence of complications were analyzed.Results After matching with 1:1 nearest neighbor matching method and propensity score matching method,there were 90 cases in the observation group and 90 cases in the control group.Compared with the control group,the operation time and probe times of the observation group were significantly lower(P<0.05),the depth of acupuncture was significantly higher(P<0.05),and the incidence of pneumothorax complications,a small amount of bleeding and total complication rate was significantly lower(P<0.05).Compared with the control group,the accuracy of diagnosis of adenocarcinoma,squamous cell carcinoma,and small cell carcinoma in the observation group was significantly improved(P<0.05).Conclusion Image fusion navigation technology guided percutaneous biopsy has high accuracy in the diagnosis of deep lung micro canceration,and has the characteristics of short operation time,small radiation dose,low complication rate,which is worthy of clinical application.
