腹腔镜根治术与传统开腹手术在结肠癌患者中的应用对比

摘　　要：目的分析腹腔镜根治术与传统开腹手术在结肠癌患者中的应用效果。方法选取2020年1月—2022年12月麻城市人民医院普外科82例结肠癌患者为研究对象,按照随机数表法分为观察组、对照组,各41例,分别接受腹腔镜根治术、传统开腹手术,并观察两组效果。结果术后,观察组手术时间、术后首次排气时间、住院时间、术中出血量优于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为2.44%,低于对照组的19.51%,差异有统计学意义(χ^(2)=4.493,P<0.05);观察组术后24、48、72 h疼痛程度评分[(5.69±1.23)分、(3.57±1.35)分、(2.21±0.05)分]均低于对照组,差异有统计学意义(t=3.353、5.666、14.593,P<0.05);观察组血清炎症因子指标水平低于对照组,差异有统计学意义(P<0.05);观察组CD4^(+)水平高于对照组,CD8^(+)指标水平均低于对照组,差异有统计学意义(P<0.05)。结论与传统开腹手术治疗对比,腹腔镜根治术在结肠癌患者中效果显著,不仅可降低各种并发症的发生以及术后疼痛感,还可改善临床各项指标,对血清免疫指标以及炎症因子具有促进作用,并且安全性较高。Objective To analyze the application effects of laparoscopic radical surgery and traditional open surgery in patients with colon cancer.Methods From January 2020 to December 2022,82 patients with colon cancer in the department of general surgery of Macheng People´s Hospital were selected as the research object,and they were divided into observation group and control group according to random number table method,with forty-one cases each.Laparoscopic radical surgery and traditional open surgery were performed respectively,and the effects of the two groups were observed.Results After surgery,the observation group had better surgical time,first postoperative exhaust time,hospital stay,and intraoperative bleeding compared to the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 2.44%,lower than 19.51%in the control group,and the difference was statistically significant(χ^(2)=4.493,P<0.05).The pain severity scores[(5.69±1.23)points,(3.57±1.35)points,and(2.21±0.05)points]of the observation group at 24 h,48 h,and 72 h after surgery were lower than those of the control group,and the difference was statistically significant(t=3.353,5.666,14.593,P<0.05).The levels of serum inflammatory factor indicators in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The CD4^(+)level in the observation group was higher than that in the control group,and the CD8^(+)index level was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with traditional open surgery,laparoscopic radical surgery has significant effects in patients with colon cancer,which can not only reduce the occurrence of various complications and postoperative pain,but also improve clinical indicators,promote serum immune indicators and inflammatory factors,and has high safety.
