摘　　要：目的:对比颞下颌关节上腔与下腔注射透明质酸(HA)联合稳定性颌垫治疗不可复性关节盘前移位的临床疗效。方法:从2014年12月至2015年8月在新疆医科大学第一附属医院颞下颌关节疾病专科门诊就诊的患者中选择72例诊断为不可复性关节盘前移位的患者纳入研究,随机分为对照组(36例)和试验组(36例)。在应用稳定性颌垫进行治疗的基础上,对照组给予颞下颌关节上腔注射HA治疗,试验组给予颞下颌关节下腔注射HA治疗,比较两组的临床治疗效果。结果:治疗后,两组患者的最大开口度和VAS疼痛评分均得到明显改善,差异具有统计学差异(P<0.05)。试验组治疗前后最大开口度变化的差值为7.19 mm,对照组治疗前后最大开口度变化的差值为4.97 mm,差异具有统计学意义(t=2.283,P<0.05)。试验组治疗前后VAS疼痛评分变化的差值为2.53分,对照组治疗前后VAS疼痛评分变化的差值为1.42分,差异具有统计学意义(t=5.199,P<0.05)。结论:对于不可复性关节盘前移位,在应用稳定性颌垫进行治疗的基础上进行颞下颌关节上腔与下腔注射HA治疗均可取得良好的治疗效果,但关节下腔注射HA的效果更好,值得在临床上推广应用。Objective:To compare the clinical efficacy of hyaluronic acid(HA)injection in superior and inferior lumen of temporomandibular joint combined with stable jaw pad in the treatment of irreducible anterior displacement of joint disc.Methods:From December 2014 to August 2015,72 patients diagnosed with irretrievable anterior displacement of joint disc were selected from the Department of Temporomandibular joint Diseases in the First Affiliated Hospital of Xinjiang Medical University,and randomly divided into the control group(36 cases)and the experimental group(36 cases).On the basis of the treatment with the stability jaw pad,the control group was given the upper cavity of temporomandibular joint injection HA treatment,and the experimental group was given the lower cavity of temporomandibular joint injection HA treatment.The clinical treatment effect of the two groups was compared.Results:After treatment,the maximum opening degree and VAS pain score of the two groups were significantly improved,and the difference was statistically significant(P<0.05).The difference of the maximum opening degree before and after treatment in the experimental group was 7.19mm,and that in the control group was 4.97mm,with statistical significance(t=2.283,P<0.05).The difference of VAS pain score before and after treatment in the experimental group was 2.53 points,and that in the control group was 1.42 points,the difference was statistically significant(t=5.199,P<0.05).Conclusion:For the anterior displacement of irreducible joint disk,both the superior and inferior lumen of temporomandibular joint can be treated by injection of HA on the basis of stability jaw pad,but the effect of injection of HA in the subarticular lumen is better,which is worthy of clinical application.
