摘　　要：目的:探究那他霉素滴眼液联合阿托品眼用凝胶治疗真菌性角膜炎的效果及对患者生活质量的影响。方法:选取2019年8月至2021年8月在广西玉林市博白县人民医院进行治疗的118例真菌性角膜炎患者作为研究对象。按照双色球法将所有患者分为氟康唑组(59例)与那他霉素组(59例)。氟康唑组患者进行结膜下注射氟康唑氯化钠注射液联合阿托品眼用凝胶治疗,那他霉素组患者进行那他霉素滴眼液联合阿托品眼用凝胶治疗,系统分析两组的临床疗效、生活质量、临床症状改善时间以及临床症状评分。结果:治疗后,相较于氟康唑组,那他霉素组患者的临床总有效率较高,P<0.05。两组患者治疗前的CQOLI-7评分与临床症状评分无显著差异,P>0.05;治疗后,相较于氟康唑组,那他霉素组患者的CQOLI-7评分较高,临床症状评分较低,P<0.05。治疗后,相较于氟康唑组,那他霉素组患者的各项临床症状(角膜溃疡、前房积脓、畏光流泪以及眼部异物感)改善时间均较短,P<0.05。结论:对真菌性角膜炎患者进行那他霉素滴眼液联合阿托品眼用凝胶治疗可显著提升其临床疗效,改善其生活质量,缓解其临床症状。Objective:To explore the efficacy of natamycin eye drops combined with atropine eye gel in the treatment of fungal keratitis and its influence on patients’quality of life.Methods:A total of 118 patients with fungal keratitis who were treated in Bobai County People’s Hospital of Yulin City,Guangxi Province from August 2019 to August 2021 were selected as the study objects.All patients were divided into fluconazole group(59 cases)and natamycin group(59 cases)according to bicolor sphere method.Patients in the fluconazole group were treated with subconjunctivium injection of fluconazole sodium chloride injection combined with atropine eye gel,while patients in the natamycin group were treated with natamycin eye drops combined with atropine eye gel.Clinical efficacy,quality of life,improvement time of clinical symptoms and clinical symptom score of the two groups were systematically analyzed.Results:After treatment,the total clinical effective rate of natamycin group was higher than that of fluconazole group(P<0.05).There was no significant difference between CQOLI-7 score and clinical symptom score before treatment between the two groups,P>0.05;After treatment,CQOLI-7 score was higher and clinical symptom score was lower in natamycin group than in fluconazole group(P<0.05).After treatment,the improvement time of various clinical symptoms(corneal ulcer,anterior chamber pus,photophobic tears and ocular foreign body sensation)in natamycin group was shorter than that in fluconazole group(P<0.05).Conclusion:Treating patients with fungal keratitis with natamycin eye drops combined with atropine eye gel can significantly improve their clinical efficacy,improve their quality of life and alleviate their clinical symptoms.
