普拉洛芬滴眼液与rb-bFGF对LASEK术后角膜修复及并发症的影响

摘　　要：目的探讨普拉洛芬滴眼液与重组牛碱性成纤维细胞生长因子(rb-bFGF)对准分子激光上皮下角膜磨镶术(LASEK)后角膜修复及并发症的预防作用。方法选择2018年1月至2022年3月南阳市第一人民医院诊治的78例LASEK术后患者(156眼)为研究对象,以随机数字表法分为A组和B组,各39例(78眼)。A组术后接受普拉洛芬滴眼液治疗,B组术后接受rb-bFGF治疗,两组均持续治疗3个月。对比两组术后刺激症状评分、基础泪液分泌量(SⅠt)、泪膜破裂时间(BUT)、角膜敏感性,对比两组术后3个月内角膜雾状混浊的发生情况。结果术后1 d,两组刺激症状评分比较差异无统计学意义(P>0.05);术后7 d,两组刺激症状评分均降低,且B组更低(P<0.05);术后1周,两组SⅠt与BUT比较差异无统计学意义(P>0.05);术后3个月,两组SⅠt与BUT均升高,且B组更高(P<0.05);术后1周,两组角膜荧光色素染色评分比较差异无统计学意义(P>0.05);术后3个月,两组角膜荧光色素染色评分均降低,且B组更低(P<0.05);术后1周,两组角膜上方、中央、鼻侧角膜敏感性比较差异无统计学意义(P>0.05);术后3个月,两组角膜上方、中央、鼻侧角膜敏感性均升高,且B组更高(P<0.05);B组角膜雾状混浊为0级例数占比高于A组(P<0.05),B组角膜雾状混浊为0.5级例数占比低于A组(P<0.05),两组角膜雾状混浊为1、2、3级例数占比比较差异无统计学意义(P>0.05)。结论相比于普拉洛芬滴眼液,rb-bFGF更有助于LASEK术后角膜修复,减缓角膜刺激症状,增加泪液分泌,提高泪膜的稳定性,且有助于预防角膜雾状混浊的发生。Objective To investigate the effect of pranoprofen eye drops and recombinant bovine basic fibroblast growth factor(rb-bFGF)aligned molecular laser-assisted subepithelial keratomileusis(LASEK)on corneal repair and prevention of complications.Methods A total of 78 patients(156 eyes)after LASEK who were admitted to the Nanyang First People’s Hospital from January 2018 to March 2022 were selected as the research objects,and divided into group A and group B by random number table method,with 39 cases(78 eyes)in each group.Group A received pranoprofen eye drops after operation,and group B received rb-bFGF after operation.Both groups continued the treatment for 3 months.The postoperative irritation symptom score,basal tear secretion(SⅠt),tear break-up time(BUT)and corneal sensitivity were compared between the two groups.The incidence of corneal haze within 3 months after surgery was compared between the two groups.Results On the 1st day after operation,there was no significant difference in the scores of irritation symptoms between the two groups(P>0.05).On the 7th day after operation,the irritation symptom scores in both groups were decreased,and the scores in group B were lower(P<0.05).One week after operation,there was no significant difference in SⅠt and BUT between the two groups(P>0.05).At 3 months after operation,SⅠt and BUT were increased in both groups,and higher in group B(P<0.05).At one week after operation,there was no significant difference in corneal fluorescein staining scores between the two groups(P>0.05).At 3 months after operation,the corneal fluorescein staining scores were decreased in both groups,and were lower in group B(P<0.05).One week after operation,there was no significant difference in the corneal sensitivity of the upper,central and nasal sides of the cornea between the two groups(P>0.05).At 3 months after operation,the corneal sensitivity of the upper cornea,central cornea,and nasal side of the two groups were increased,and the sensitivity of group B was higher(P<0.05).The proporti
