凝血酶冻干粉在超声引导下治疗医源性假性动脉瘤的效果观察

摘　　要：目的:观察超声引导下医源性假性动脉瘤瘤腔内注射凝血酶冻干粉治疗医源性假性动脉瘤的临床效果。方法:选取2018年1月—2022年12月于绵阳市中心医院行股动脉穿刺后并发假性动脉瘤且常规压迫治疗无效患者39例。观察超声引导下瘤腔内注射凝血酶冻干粉治疗的临床效果,并观察有无下肢动脉血栓、瘤腔破裂等并发症。结果:35例患者一次性治疗成功,3例患者术后24h第2次治疗成功,瘤腔闭塞,1例患者经3次注射效果不佳改行外科手术治疗,总治愈率约97.4%。结论:超声引导下医源性假性动脉瘤瘤腔内注射凝血酶冻干粉治疗医源性假性动脉瘤,操作方便,成功率高,值得临床应用。Objective To observe the clinical effect of ultrasound-guided intracavitary injection of thrombin lyophilized powder into iatrogenic pseudoaneurysm.Methods A total of 39 patients with pseudoaneurysm after femoral artery puncture in Mianyang Central Hospital from January 2018 to December 2022 were selected,and the conventional compression treatment was ineffective.The clinical effect of thrombin lyophilized powder injected into the tumor cavity under the guidance of ultrasound was observed,and the complications such as lower limb artery thrombosis and aneurysm cavity rupture were observed.Results 35 patients were successfully treated at one time,3 patients were successfully treated twice within 24 hours after operation,the tumor cavity was occluded,and 1 patient was converted to surgical treatment after 3 injections with a total cure rate of 97.4%.Conclusion Ultrasound-guided intracavitary injection of thrombin lyophilized powder into iatrogenic pseudoaneurysm is minimally invasive,easy to operate and has a high success rate,which is worthy of clinical promotion.
