摘  要:目的探讨4种常用甜味剂对七味白术散治疗抗生素相关性腹泻的影响,为药剂辅料的选用对药效的影响及临床治疗指导患者服药提供实验依据。方法选择42只SPF级昆明种小鼠随机分为正常组、模型组、七味白术散组、蔗糖组、糖精钠组、山梨醇组和木糖醇组,每组6只。造模阶段正常组灌胃无菌水,模型组和治疗组采用抗生素混合液(庆大霉素+头孢拉定)建立小鼠抗生素相关性腹泻模型。治疗阶段对正常组和模型组小鼠灌胃无菌水,5个治疗组小鼠分别灌胃七味白术散、蔗糖+七味白术散、糖精钠+七味白术散、木糖醇+七味白术散及山梨醇+七味白术散。治疗结束后,分别收集各组小鼠脾脏、胸腺以及肠道内容物,进行脏器指数、肠道微生物数量及肠道酶活性分析。结果与模型组相比,七味白术散组和山梨醇组小鼠的脾脏指数增加,但差异无统计学意义(均P>0.050);各组小鼠的胸腺指数差异无统计学意义(P>0.050)。与七味白术散组相比,添加甜味剂组别的腹泻小鼠肠道内容物中的细菌数和大肠埃希菌数均有一定程度的增加,其中山梨醇组的大肠埃希菌数量变化最小(P=0.002);蔗糖组和山梨醇组腹泻小鼠肠道内容物中的双歧杆菌数量(P<0.001、P>0.050)和乳杆菌数量(P=0.001、P<0.001)均有所增加。与七味白术散组相比,添加甜味剂组别的小鼠均在不同程度上提高了酶活性,其中木糖醇组酶活性最高,山梨醇组和蔗糖组次之,糖精钠组酶活性最低。结论甜味剂的添加对七味白术散治疗抗生素相关性腹泻疗效有一定影响,山梨醇作为甜味剂在药物制剂领域中的应用十分具有前景,但其是否能够替代传统甜味剂蔗糖在未来中药制剂中作为矫味剂被选用仍需进一步进行探究。Objective To observe the effects of four commonly used sweeteners on the treatment of antibiotic-associated diarrhea with Qiwei Baizhu Powder(QWBZP),and provide an experimental basis for the choice of pharmaceutical excipients and clinical guidance for patients to take the drugs.Methods Forty-two SPF-grade Kunming mice were randomly divided into normal group,model group,QWBZP group,sucrose group,sodium saccharin group,sorbitol group and xylitol group,with six mice in each group.During the modeling,sterile water was gavaged in the normal control group,and antibiotic mixture(Gentamicin+Cefradine)was used in the model and treatment groups to establish the antibiotic-associated diarrhea model.During the treatment,the normal and model groups were gavaged with sterile water,and the other five treatment groups were gavaged with QWBZP,sucrose+QWBZP,sodium saccharin+QWBZP,xylitol+QWBZP and sorbitol+QWBZP respectively.After the treatment,the spleen,thymus and intestinal contents of mice in each group were collected for analysis of organ indices,intestinal microbial counts and intestinal enzyme activities.Results Compared with the model group,the spleen indices in the QWBZP and sorbitol groups increased,but with no significant differences between groups(P>0.050);the thymus indices in all groups were not significantly different(P>0.050).The counts of bacteria and E.coli in the intestinal contents of diarrhea mice with ingestion of sweetener increased to some extent compared with the QWBZP group,with the smallest change in the count of E.coli in the sorbitol group(P=0.002);Bifidobacterium(P<0.001,P>0.050)and Lactobacillus(P=0.001,P<0.001)increased in the intestinal contents of the diarrhea mice in the sucrose and sorbitol groups.The enzyme activity increased to different degrees in all the groups with addition of sweeteners compared to the QWBZP group,with the highest enzyme activity in the xylitol group,followed by the sorbitol and sucrose groups,and the lowest enzyme activity in the sodium saccharin group.Conclusion The add
