摘  要:目的探究血清氨基末端脑钠肽前体(NT-pro-BNP)、氧化型低密度脂蛋白(ox-LDL)水平与TIA患者并发脑梗死(CI)的关系。方法选取2016年10月至2020年1月本院诊治的141例TIA患者为TIA组,另选取同期141名健康体检者为对照组。ELISA测定血清NT-pro-BNP、ox-LDL水平。按ABCD2评分将TIA患者分为低危组(n=48)、中危组(n=50)、高危组(n=43),比较三组血清NT-pro-BNP、ox-LDL水平;按TIA患者90 d内是否发生CI将其分为未并发CI组(n=102)和并发CI组(n=39),比较两组TIA患者一般资料及血清NT-pro-BNP、ox-LDL水平;Logistic回归分析研究TIA患者并发CI的影响因素。结果TIA组患者血清NT-pro-BNP、ox-LDL水平明显高于对照组(均P<0.05)。随着TIA疾病加重,TIA患者血清NT-pro-BNP、ox-LDL水平呈升高趋势(均P<0.05)。与未并发CI组相比,并发CI组TIA患者糖尿病史、临床症状持续时间≥60 min、高血压史的比率及血清NT-pro-BNP、ox-LDL水平均明显升高(均P<0.05)。NT-pro-BNP、ox-LDL与TIA患者并发CI相关(均P<0.05)。结论NT-pro-BNP、ox-LDL在TIA患者血清中水平较高,二者均与TIA患者病情严重程度、TIA并发CI相关,均有望成为评估TIA患者病情严重程度的指标。Objective To explore the relationship between serum N⁃terminal pro⁃brain natriuretic peptide(NT⁃pro⁃BNP),oxidized low⁃density lipoprotein(ox⁃LDL)levels and cerebral infarction(CI)in patients with TIA.Methods A total of 141 TIA patients diagnosed and treated in our hospital from October 2016 to January 2020 were selected as the TIA group,and 141 healthy patients during the same period were selected as the control group.ELISA was used to determine serum NT⁃pro⁃BNP and ox⁃LDL levels.According to ABCD2 score,TIA patients were divided into low⁃risk group(n=48),intermediate⁃risk group(n=50),and high⁃risk group(n=43),serum NT⁃pro⁃BNP and ox⁃LDL levels were compared in the three groups.TIA patients were divided into non⁃CI group(n=102)and complicated CI group(n=39)according to whether CI occurred within 90 d,and the general data and serum NT⁃pro⁃BNP,ox⁃LDL levels were compared in the two groups.Logistic regression analysis was used to analyze the influencing factors of concurrent CI in TIA patients.Results Serum NT⁃pro⁃BNP and ox⁃LDL levels in the TIA group were significantly higher than those in the control group(all P<0.05).As the TIA disease worsened,the serum NT⁃pro⁃BNP and ox⁃LDL levels in the TIA patients showed an increasing trend(all P<0.05).Compared with the uncomplicated CI group,the proportion of TIA patients with a history of diabetes mellitus,the proportion of clinical symptoms lasting≥60 min,the proportion of history of hypertension,and the serum NT⁃pro⁃BNP and ox⁃LDL levels were significantly increased in the complicated CI group(all P<0.05).NT⁃pro⁃BNP and ox⁃LDL were associated with concurrent CI in TIA patients(all P<0.05).Conclusion The levels of NT⁃pro⁃BNP and ox⁃LDL in the serum of TIA patients are relatively high,both of which are related to the severity of TIA patients and the concurrent CI in TIA patients,and are expected to be indicators for evaluating the severity of TIA patients.
